Panel Meeting Evaluation

The Wisconsin Arts Board asks all panelists for feedback following their service on one of our grant review panels. Please fill out this form with detail. While this form is not anonymous, we ask that you do not shy from constructive criticism. Your perspective informs us, and allows us to improve our grant process.
