    Terms and Conditions Agreement:The below Terms and Conditions are acknowledged and agreed to by and between the Parent/Guardian and The Crafty Crow NM Fall Break Camp(the “Camp”) and the undersigned parent or guardian (referred to herein as “Parent”, “Guardian”, “I”,“Me”, “You” and “We”) on behalf of Parent/Guardian’s child (the “Camper”) and represent the expectations and agreements in order to be eligible for acceptance to Camp. Terms and Conditions consist of the following: Requirements for Acceptance to Session at CampPayment, Refund and change of policies Camper’s Participation and Camp Program Expectations Campers Safety Parent/Guardian AuthorizationsPhoto ReleaseRelease and Indemnification Requirements for Acceptance to Session at Camp:• In the mutual interests of Camp, Camper, and for the well-being of all campers at Camp,I understand that Camper must respect and observe the rules, regulations and policiesof Camp, which have been developed for the benefit of all campers including Camper tostrengthen the Camp program and enhance the experience for all campers. In particular,as Parent/Guardian of Camper, I agree to the implementation and enforcement of therules, regulations and policies at Camp for the Fall Break Camp Session, and whichare part of this Agreement, and have reviewed these rules, regulations and policies withCamper.• I agree that Camper’s acceptance into a session at Camp is not confirmed until explicitwritten verification is received from the Camp, which notice will usually come fromCamp via email to the email address established by me in my application.• I understand that The Crafty Crow NM  consider Camp safety and public health matters of utmost importance. The health of all members of thecamp community ranks as a key component in maintaining a safe environment and indecreasing the risk of transmission of contagious illness. Accordingly, The Crafty Crow NMrequire that any member of the camp community is well when attending camp.  No fevers, communicable diseases or illnesses are present. Health Forms.• I agree, prior to the beginning of the camp, to check for any symptoms that may prevent my child from attending.I will note any and all allergies.I will note any major medical, physical, developmental, social-emotional challenges my child may have so that they can be managed as is appropriate for your child.  Deposit, Payment, Refund and Change of Session Policies:Deposit• I agree to submit at time of registration via electronic or cash payment method (Credit Card,Debit Card, or Venmo) a $80 per Camper/per day or Sliding Scale $50 per Camper/per day  NON-REFUNDABLE payment in order to have Camper’s application processed. Camper’s Participation and Camp Program Expectations:• I agree to have Camper in attendance at Camp on-time and for the full extent ofCamper’s scheduled session. Accommodations for late arrival, early departure and/orexcused absence must be made in writing (text) in advance of the scheduled session, and willonly be granted when possible, with written permission of the Camp. •I agree to pack my child a lunch, refillable water bottle each day of camp.• I agree to send my child in warm layers in case of temperature/weather fluctuations as part of each day is spent outside.• I understand that I, as Parent/Guardian am responsible for all transportation to andfrom Camp.• I hereby give permission and acknowledge the expectation for Camper to engage in allactivities (except as noted on the Health Form and acknowledged by Camp)..Camper Safety• The Crafty Crow NM Director and staff must pass annual background checks• Camps are MANDATED REPORTERS of child abuse.o If a camper reports harm of physical, sexual, or emotional nature (whether at home, outside of camp, or at camp), staff is required to report concerns to theappropriate authorities. Parent/Guardian Authorizations:• I/We give permission to Camp to provide routine health care, and seek emergencymedical treatment including but not limited to ordering x-rays and routine tests. I/Wegive permission for Camp to release any of Camper's and our medical and insurance card.•In case of Camper’s medical emergency, I/We hereby give permission for an ambulance to be called and a physician selected by the Hospital to secure proper treatment for, and to order injection,anesthesia or surgery for Camper. Every effort will be made by Camp to immediatelycontact You in the event of any such emergency. Information may be printed/photocopied for access in an emergency involving Camper.• I/We authorize the Camp staff to administer over- the- counter medications,medicines prescribed for Camper, including medications that Camper brings to Camp,those listed as medications Camper is currently taking, and medications needed in thecare and treatment of Camper.• I/We authorize the Camp staff to administer life saving or basic medical assistance (First Aid/CPR certified) Photo Release:Except as set forth in the Parent/Guardian Authorizations above, Parent/Guardian also herebyconsents to the unlimited photography for use by campers to make their photo albums at the end of the week and for future camp promotions. Release and Indemnification:Release:• I understand that Camp accepts no responsibility for loss or damage to Camper’sproperty (including clothing, equipment, personal effects or money) incurred during thesession at Camp or while in transit. Camp highly recommends that audio equipment,computer games, phones, money or items with significant material value or sentimentalvalue not be brought to Camp. Although every effort is made to return lost or misplaceditems, Camp cannot be held responsible for Camper’s misplaced, lost or stolen property.• In the mutual interests of Camp, Camper and other campers, Parent/Guardian willrespect and observe the rules, regulations and policies of Camp as laid out in this agreement. Agreement,• I understand that Camp reserves the right to amend/update these Terms and Conditionsand any Rules and Polices as necessary Indemnification:Camper and Parent/Guardian, for Camper and themself(ves) hereby release, discharge, andhold harmless The Crafty Crow NM Camp, the The Crafty Crow NM and their employees, officers, directors and trustees from any andall claims, expenses, costs, damages, fees (including attorneys’ fees), lawsuits, or other legal causes of action, resulting from and/or relating to Camper's participation in a The Crafty Crow NM Program Camp, Camp events and programs, and Camper's travel to and from Camp.
