1. Definitions & Covered Entities
For purposes of this agreement:

“DCHL/MRS” refers to DC Hockey Leagues LLC, Maryland Recreational Sports LLC, and any program, clinic, camp, league, or event operated under the name “District Ball Hockey,” which is used as a trade name by DCHL/MRS.
This waiver applies equally to all activities offered or operated by DCHL/MRS, regardless of which name or branding is used.

2. Voluntary Participation & Assumption of Risk
I acknowledge that participation in activities offered by DCHL/MRS involves inherent risks, including but not limited to: slips, falls, collisions, contact with boards, flooring or playing surfaces, other participants, equipment, pucks/balls, sticks; injuries related to skating, running, training, or physical exertion; and other hazards associated with hockey, ball hockey, roller hockey, open gym, training sessions, or recreational sports.

I voluntarily accept and assume all risks of injury, illness, property damage, or death, whether known or unknown.

3. Release of Liability (Including Ordinary Negligence)
In consideration for being allowed to participate, I hereby release, waive, and discharge DCHL/MRS, Rockville Sports Arena, their owners, agents, employees, contractors, officials, and volunteers from any and all liability, including injuries resulting from the ordinary negligence of the parties listed above.

I understand that this release does not waive claims for gross negligence, as prohibited by Maryland law.

4. Indemnification
I agree to indemnify and hold harmless DCHL/MRS from any claim, demand, or lawsuit brought by me, my child, or any third party arising from:

my participation or my child’s participation
my actions or my child’s actions
my failure to follow rules or instructions
any damage or injury caused by me or my child
This includes attorney’s fees and court costs.

5. Medical Consent
In the event of injury, illness, or emergency, I authorize DCHL/MRS to:

obtain medical care,
contact emergency services, and
transport me or my child if necessary.
I accept full financial responsibility for all medical expenses.

6. Photo/Video/Media Release
I grant DCHL/MRS unrestricted permission to use photographs, film, or digital images of me or my child for advertising, social media, promotional, or lawful purposes, without compensation.

7. Concussion & Head Injury Acknowledgment
I understand the risks of concussions and head injuries in hockey and related sports.
I agree to follow all league concussion protocols, including removal from play if a concussion is suspected.

8. Equipment & Facility Acknowledgment
I acknowledge and accept that:

All equipment and playing surfaces carry risks.
Boards, walls, rink fixtures, and surrounding structures may contribute to injuries.
I am responsible for ensuring that my (or my child’s) equipment is safe and properly fitted.

9. Communicable Disease & Illness Waiver
I acknowledge the risk of exposure to COVID-19 and other communicable diseases and voluntarily assume all such risks.
I release DCHL/MRS from liability related to any illness contracted during participation.

10. No Refunds / No Guarantees of Participation
I understand and agree that:

All fees paid are non-refundable.
Program changes, injuries, team changes, or schedule adjustments do not entitle me to a refund or credit.
Signing this waiver does not guarantee any specific playing time, team placement, or position.

11. Electronic Signature Consent
By submitting this form online, I agree that:

My electronic signature has the same legal effect as a handwritten signature.
I am signing this document knowingly and voluntarily.

12. Governing Law, Venue & Jurisdiction
This agreement shall be governed by the laws of the State of Maryland.
Any dispute, claim, or legal action shall be brought exclusively in the state courts of Montgomery County, Maryland.

13. Severability
If any provision of this agreement is found unenforceable, the remainder shall continue in full force and effect.

14. Binding Effect
This agreement is binding upon:

myself,
my heirs,
my executors,
my administrators,
my assigns, and
in the case of a minor participant, the minor and the minor’s legal representatives.

ACKNOWLEDGMENT & SIGNATURE
By signing below, I acknowledge that:

I have read and fully understand this waiver;
I am voluntarily giving up substantial legal rights, including the right to sue for injuries resulting from ordinary negligence;
I agree to all sections above in their entirety;
I understand this agreement covers programs operated under DC Hockey Leagues, Maryland Recreational Sports, and District Ball Hockey.
