Aroma Social Participant Waiver and Release Form

Welcome to our Create Your Own Perfume Experience! This waiver form ensures your understanding and agreement to participate safely. Please read all sections carefully before signing.

I understand that I am voluntarily participating in a Create Your Own Perfume experience hosted by Aroma Social. This activity involves handling fragrance materials, essential oils, alcohol bases, glass containers, and other substances that may have strong scents or cause irritation if used improperly.

I acknowledge that fragrance and cosmetic ingredients may cause skin irritation, allergic reactions, dizziness, or respiratory sensitivity; Aroma Social provides safety instructions and ingredient lists, but cannot guarantee an allergen-free environment; I am responsible for informing Aroma Social of any known allergies, sensitivities, or medical conditions before participating; I am voluntarily assuming all risks, both known and unknown, associated with participation in this activity.

I understand that working with fragrance oils and ingredients may involve risks such as skin irritation, allergic reactions, or accidental spills. I agree to follow all safety instructions and notify staff immediately if I experience any discomfort.

I agree to follow all safety instructions provided by Aroma Social staff, use materials and equipment as directed, avoid ingestion or direct contact with undiluted oils or alcohol bases and refrain from participating if I feel unwell or experience discomfort during the activity.

In consideration of participating, I hereby release, waive, and forever discharge Aroma Social, its owners, employees, contractors, and affiliates from: Any and all claims, damages, liabilities, or costs arising from injury, illness, or loss that may occur as a result of my participation; Any claims relating to allergic reactions, staining of clothing, or accidental damage to personal property. I understand this waiver applies to myself, my heirs, and any personal representatives.

I acknowledge that Aroma Social staff are not medical professionals and cannot provide medical advice. I agree to seek medical attention immediately if I experience an adverse reaction. Aroma Social wil not be monetarily responsible for any medical bills if deemed necessary.

If you do not agree to the social media and photography permission, please inform our staff before beginning the experience.

I understand that: The perfume or product I create during this session is for personal use only and not for resale; Aroma Social is not responsible for how I store or use the final product after the event; Fragrance composition and shelf life may vary based on materials and handling.
