SCHOLARSHIP APPLICATION FORM

This official packet that allows you to be considered for the FUNDET Scholarship program at alt Lake, Mountainland, and Ogden-Weaver Technical College. Please read the instructions carefully to assure that the scholarship criteria, requirements, and guidelines are standards that you can abide with and meet your expectations. To be considered for the scholarship you must meet the following criteria:
- Must have the math and English proficiency to be admitted into the program.
- Be a Hispanic/Latino adult enrolled in a technical training to enter the workforce.
- Commit to complete the training program.
- Pay between 25% to 50% of the program tuition and fees plus books and supplies.
- Maintain 90% attendance and completion for the length of the program.
- Commit to 10 hours of community service a month for the length of the program.
- Student will attend a Monthly Progress Interview with a FUNDET mentor.
