    Before beginning, make sure you have the following information:

    • Contact information for an emergency contact (not a parent)
    • Your student's t-shirt size and sandwich choice (ham, turkey, chicken salad, or vegetarian)
    • Your student's most recent grade report
    • Your student's health insurance ID card
    • Letter of recommendation from teacher/counselor/principal (can be uploaded now or submitted online by recommender) http://educationinaction.org/youth-facilitator-recommendation
    • Digital headshot photo (does not need to be professional) to go with your Youth Facilitator bio on Education in Action's Lone Star Leadership Academy Staff page, https://educationinaction.org/lone-star-leadership-academy/staff

     

    In addition, this form includes six essay questions to which you should be prepared to respond. There are four student questions and two parent questions.

    Student Questions:

    1. Why do you want to be a Youth Facilitator?

    2. What do you hope to gain from this experience?

    3. What are your strengths and weaknesses as a leader?

    4. What do you enjoy most about the Lone Star Leadership Academy?

    Parent Questions:

    1. What do you believe your student will contribute to the Lone Star Leadership Academy as a Youth Facilitator?

    2. What do you hope your student will gain from this experience?

    This should be someone other than a parent or guardian.
  • Consent to Release Information

  • At the end of each session, campers and youth facilitators receive a camp directory to help them stay in touch with new friends. Students are included only with parent/guardian permission. The directory lists each student’s name and cell phone and/or email address. If a student doesn’t have a cell phone or email, parents often provide their contact information instead. The directory is shared only with campers and youth facilitators from the same session and is never sold or shared outside the group.

  • Letter of Recommendation

  • Youth Facilitator applicants must submit a letter of recommendation written by a teacher, counselor, or principal, which addresses the applicant's suitability to serve as a mentor to younger students.

    You may upload a copy of your letter here, or your recommender can submit the letter online themselves. You will receive an automatic email upon submission of your application that includes instructions to pass along to the individual writing your recommendation letter if he/she is going to submit it online.

  • Health insurance is required for participation in the Lone Star Leadership Academy. If your student does not currently have medical insurance, Education in Action can provide you with information about short-term travel insurance. If you are not able to complete the acknowledgement below, please call our office at 817-562-4957 ext. 104 before proceding.

     


     LONE STAR LEADERSHIP ACADEMY

    HEALTH INSURANCE ACKNOWLEDGEMENT

    Education in Action requires all Lone Star Leadership Academy participants to have health insurance for the duration of their program.

    Education in Action will provide information about a low-cost travel insurance plan which provides emergency medical coverage during the program and meets the requirements of the Lone Star Leadership Academy. You are also welcome to find your own source of coverage.

    Education in Action will email you approximately four weeks prior to the start of your student's camp with instructions for purchasing the travel insurance plan or providing proof of coverage through another source.
