Waiver of Liability and Release of Indemnification
I understand that although organizers of the Christian Youth Camp would like everyone to have a safe experience, I acknowledge that the inherent risks cannot be eliminated. Such risks include, but are not limited to, boating, swimming, tubing, hiking, rope climbing, and other sports or activities that involve inherent risk. Aware of these risks and willing to assume them, I hereby waive, release, and agree to hold the owners of Spirit in the Pines Camp and Word of Life Church and their representatives and successors for all claims or liabilities of any kind arising out of my minor child’s participation in this camping experience. I understand that Spirit in the Pines Camp and Word of Life Church assumes no responsibility for injuries or illness which my minor child may sustain because of any physical condition or resulting from participation in any camp activities or experiences. I hereby release and discharge Spirit in the Pines Camp and Word of Life Church and their directors, officers, employees, and volunteers from all claims for accidents, injuries, death, loss, or damage which my minor child may suffer because of participation in these activities.
If my minor child needs immediate medical attention for injuries received while participating in the camp programs, I authorize Spirit in the Pines Camp’s and Word of Life Church’s representatives to give my minor child reasonable first aid and to arrange transport of my minor child to a health care facility for medical services, as needed. I authorize Spirit in the Pines Camp’s staff and representatives of the Word of Life Church to administer medication when instructed to do so by a poison control center. If my child requires the use and administration of any oral medication or injections (use of syringes), I understand that it is my responsibility to ensure that the needed medications/devices are on my child or within their personal belongings every day of the camp program and they know what to take and when. If Spirit in the Pines Camp’s staff or Word of Life representatives are required to administer medications or use syringes, I agree, release, and discharge all of Spirit in the Pines Camp staff and Word of Life Church representatives, its directors, officers, employees, and volunteers from all liability arising out of or resulting from the use or administration of medications, including medical syringes. I hereby acknowledge that Spirit in the Pines Camp and Word of Life Church will assume that either parent of the minor child may pick up the child at any time during the program unless there is pertinent court documentation on file at Spirit in the Pines Camp and Word of Life that indicate otherwise.
