I hereby grant Looking Good Girl (the “Releasee”) permission to use my photographs and/or likeness in any of the following formats, without limitation:

Web-based publications and social media platforms
Print advertisements and marketing materials
Organizational bulletins or internal publications

I acknowledge and agree that this authorization is granted voluntarily and without expectation of compensation or royalties of any kind, now or in the future.

I understand that all photographs and related materials shall become the exclusive property of the Releasee and may be used, reproduced, or distributed at their discretion, provided such use respects my personal rights as the subject of the photographs.

I hereby waive any right to inspect or approve the finished product, including written or electronic copies, in which my likeness appears.

I affirm that I am of legal age and have the full right to contract in my own name. In the event that I am under the age of consent, I confirm that I have obtained the necessary permission from my parent(s) or legal guardian(s), as evidenced by their signature(s) below.

Furthermore, I release, discharge, and hold harmless Looking Good Girl, its officers, employees, representatives, and affiliates from any and all claims, demands, or causes of action that I, my heirs, executors, administrators, or any other persons acting on my behalf or on behalf of my estate may have arising out of or in connection with the use of the photographs or likenesses authorized herein.
