    Cottage Lake Elementary School Student Registration
  • The Cottage Lake PTA is excited to bring Missoula Children's Theater (MCT) back to Cottage Lake Elementary for this year's play, Cinderella!  They will bring the costumes, set, props, and directors, we bring the students!

    **Due to popularity of this program we will be capping the sign ups according to the allocated number of roles for each age group, per MCT guidlines:

    • x16  Ages 7 and under
    • x44  Ages 8-12
    • x4    Assistant Director ages 10-12 

    **Please also remember - your student(s) are required to be available and attend ALL rehearsals and BOTH Saturday performances.  

    The PTA loves making activities accessible to all, so there is no cost for students to participate in this weeklong opportunity.

    *If you need to withdraw your student(s) registration, please do so at your earliest convenience by January 11th by emailing theater@cottagelakepta.org.

            • Parent/Guardian Information

            • Cottage Lake Elementary PTA Medical Release

              For every child participating in this after-school program, all parents/guardians are required to fill out the medical release information below. 


              NOTE: The CLE PTA does not have access to medications or supplies in the school office nor can PTA volunteers administer prescription or over-the-counter medications. Thus, if your child requires any medications (including inhalers, prescriptions, epi-pens, etc) you must provide and administer these, if needed. PTA volunteers will be supplied with basic First Aid supplies and in the event of an emergency will make use of the 911 system for assistance.

            • Emergency Contact Information

              In case of a serious medical emergency, 911 will be called to evaluate your student.  Please provide two phone numbers where someone can be reached during after school program hours:

            • Participation Requirements

              1. All parents/caregivers must arrive by 6:00pm on Monday, January 12th with a sack dinner in case their student needs to stay for the second part of rehearsals (6-8pm) that evening.


              2. All cast members MUST be available for ALL required role rehearsals; students may have to skip other commitments that week to ensure they are available.


              3. All cast members MUST be available for both performances on Saturday, January 17th.

              4. Parents/Caregivers must pick up students from the gym after the audition and/or rehearsals. No student will be allowed in the parking lot to wait for pick-up. If your child is a walker, they still need to be checked out by an adult who then takes the responsibility of ensuring it is the proper time of day for walking and decides that walking conditions are safe.

              5.  I acknowledge that by participating in the Cottage Lake Elementary Missoula Children’s Theatre production; Cinderella, my child will be video recorded and photographed. 

              6. The professional recording is for personal use only and may not be shared publicly (including YouTube, Facebook, and/or any other social media platforms). Professional videos may be purchased at the time of the show and the USB will be delivered to your child at school the week following the production.

            • Transportation Consent

              Parents will need to notify CL Attendance & their Classroom teacher via email or written note that their child will forego their normal means of after-school transportation during the week of January 12th-17th to participate in Missoula Children's Theater activities. Children will be released from their class to morning recess immediately following the end of the class.

            • Informed Consent and Acknowledgement

              I hereby give my approval for my child’s participation in any and all activities prepared by Cottage Lake Elementary PTA during Missoula Children's Theater activities. In exchange for the acceptance of said child’s candidacy by Cottage Lake Elementary, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Cottage Lake Elementary PTA, Missoula Children's Theater, and all respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from Missoula Children's Theater sessions.

              In signing this form, I acknowledge that instructors/PTA volunteers cannot and will not administer medication and all such medication will be provided and administered by a parent or guardian if necessary and that my student is hereby granted permission to participate in enrichment activities (before or after school) sponsored by the Cottage Lake Elementary PTA.
