· Please bring a cell phone with you on any trip. Make sure the office has the number, and in case of emergency, please notify the school immediately at 928-445-6306.
· As volunteers and chaperones, you are acting as a Christian role model for our students. Please be an example by demonstrating maturity in actions, attitudes, and dress.
· No tobacco products in the presence of, or within sight of students. You may not use, sell, provide, possess, or be under the influence of drugs or alcohol.
· You may not be in the possession of any weapon.
· A classroom teacher will be in charge of each field trip and will direct the volunteers as to what is expected of you.
· Chaperones are responsible for the behavior of the students in their private vehicles.
· Teachers are responsible for student behavior (with chaperone help) in school-owned or rented vehicles.
· For the duration of the field trip, chaperones are responsible for the group of students assigned to them. Please monitor behavior, keep count of your group continually, and report difficult behavior to a teacher or supervisor immediately.
· Behaviors not tolerated by Trinity Christian School from students on a field trip include: rowdiness, disrespect, inappropriate voices, dangerous or potentially dangerous behavior, public/private displays of affection (PDA).
· Students are to return with the same person and vehicle that transported them to an event. Emergency exceptions may only be made by an administrator or sponsoring teacher.
· Drivers and chaperones should refrain from purchasing special treats for their group of students.
· Teachers will make vehicle assignments.
· Chaperones are asked to arrive at school at least 15 minutes prior to departure time and check in with the office.
· Chaperones will be given copies of Emergency Medical Release forms for all students to keep in their vehicle.
· Seat belts must be worn at all times. Only one child is permitted per working seat belt.
· Chaperones are expected to take the route given by the teacher, and to “caravan” or “buddy system.” Drive together, so long as it is safe. Please do not pass the lead car.
· Chaperones are to obey all traffic laws, including maintaining acceptable speed limits.
· Students must enter and leave the vehicle from the curbside unless the vehicle is in a protected parking area or driveway.
· Students may not be left unattended in a vehicle.
· Volunteer Drivers should expect no payment for transportation or fuel costs.
Drivers, please submit your current driver’s license and proof of insurance to be copied by the office.
This application must be submitted no less than 5 business days prior to the event you are applying for and will be good for one school year.
I have read and agree to abide by the chaperone/driver guidelines given by Trinity Christian School. I understand that a Limited Criminal Background and Motor Vehicle Record Check will be completed and kept on file with this form. This form is only valid for three years and must be updated when a new background check is required.
