TMA – LIABILITY WAIVER

I understand football training involves risk. I release TMA from liability.

I authorize TMA to use photos/videos of my athlete for training & media. ( Y/N )

All payments must be made in full before training. No refunds. No Exceptions.

• Respect all coaches and teammates.

• No fighting, cursing, or unsafe behavior.

• Parents must maintain positive sideline behavior.

• Bring water, cleats, and full effort.

• Arrive 15 minutes Early.

• No Refunds for missed sessions!

• 2 Make up session allowed per month.

• Parent ARE NOT allowed to intervene, interact, or disrupt any session unless for emergency reasons. 

• I agree not to hold TMA, coaches, staff, or facility owners responsible for accidental injuries.

• TMA is not responsible for athletes outside session times.

• Fighting, Disrespect, Stealing, Cursing, and Constent Lateness Are all susceptible To PERMANENT Termination From The Program.
