I understand and acknowledge the following regarding the nature of "Eyden Studio™": Informal Status: I know that "Eyden Studio™" is a personal creative credit name for independent projects run by Eyden Villarreal, a student. It is not a registered business, formal organization, non-profit, or legal employer. Volunteer Nature: My child is participating strictly as an unpaid volunteer. This arrangement is for mutual creative development and project completion, and my child will not receive monetary compensation or employee benefits. Supervision: I understand that supervision will be peer-to-peer and project-focused, and my child will be working alongside Eyden Villarreal and/or other student volunteers. I acknowledge that formal, adult-level organizational supervision may not be provided.

Emergency Medical Authorization (Texas Requirement)

In the event of an injury or illness, and if I (the parent/guardian) cannot be reached immediately after a reasonable effort has been made, I authorize Eyden Villarreal or any supervising adult (if applicable) to secure and arrange for emergency transportation and/or medical care, including non-surgical medical treatment, necessary for my child's immediate welfare. I agree to be responsible for any medical bills incurred.

Waiver and Release of Liability

This must be checked off and read by the parents/guardian(s).
I understand that participation in volunteer activities carries inherent risks, both foreseen and unforeseen, including but not limited to risks associated with travel, technical work, and general physical activity.DEFINITIONS: "Released Parties" shall collectively mean: Eyden Villarreal (as the individual behind Eyden Studio™), his parents/guardians, immediate family members, agents, and any other individuals officially collaborating on Eyden Studio™ projects." Releasing Parties" shall collectively mean: the undersigned Parent/Guardian, the Youth Volunteer, and their respective heirs, personal representatives, and assigns. A. WAIVER AND RELEASE OF LIABILITY (Covenant Not To Sue):As the parent/legal guardian, for myself and on behalf of the Releasing Parties, I hereby fully release, waive, discharge, and covenant not to sue the Released Parties from any liability, claims, demands, actions, and causes of action whatsoever, whether known or unknown, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or my property while participating in, or traveling to or from, any Eyden Studio™ project activity.B. INDEMNIFICATION:I further agree to indemnify (hold harmless and defend) the Released Parties against any claims, demands, or suits, including all associated defense costs and attorney fees, brought by the Releasing Parties, or by anyone claiming to be acting on behalf of the Releasing Parties (including my child), which arises out of my child’s participation in Eyden Studio™ activities, even if such claim is alleged to be the result of negligence by the Released Parties. This Agreement shall be governed by the laws of the State of Texas.*

I acknowledge that I have read and understand the nature of the activities my child will be undertaking and the comprehensive terms of this agreement.

Project Logistics, Travel, and Technology Consent

I consent to the following logistics required for my child’s volunteer participation:
Travel and Transportation: I authorize my child to travel with Eyden Villarreal, another designated student volunteer, or their parent/guardian, or to use public transportation, for purposes directly related to Eyden Studio™ projects (e.g., meeting at a library, attending a local event). This consent includes transportation to and from project meeting locations.*

YES, I authorize project-related travel.NO, I limit travel to drop-off/pickup by myself only.
Meeting Locations: I authorize my child to meet for collaborative project work at public, supervised locations (e.g., libraries, coffee shops) and/or private residences of other student volunteers or the home of Eyden Villarreal, provided I am informed of the date and time in advance.*

Use of Online Platforms & Tools: I grant permission for my child to use common online collaboration tools necessary for project management, which may include Discord (for communication), Google Docs/Drive, GitHub/GitLab, or other similar platforms. I acknowledge that I am responsible for supervising my child’s use of these platforms.*

Intellectual Property and Work Contribution
I understand that my child will be contributing original work (e.g., art, code, writing, design) to collaborative projects under the Eyden Studio™ name. Credit and Attribution: My child will always be credited for their specific contribution to a project. Eyden Villarreal, under Eyden Studio™, retains the right to use the completed, collaborative project for non-commercial portfolio use, display, or entry into student competitions. Waiver of Further Compensation: I acknowledge that my child’s contribution is made as a volunteer. Should the finished project ever generate revenue (e.g., winning prize money, minor digital sales), I waive any claim to present or future financial compensation beyond receiving clear credit for my child’s work. Right to Complete and Display: I grant Eyden Villarreal (Eyden Studio™) the necessary, non-exclusive rights to integrate, modify, and use my child’s contribution solely for the purpose of completing and presenting the final collaborative project.*

Student Volunteer Code of Conduct Acknowledgment
The detailed Eyden Studio™ Student Volunteer Code of Conduct is provided as a separate document that outlines specific expectations for behavior, safety, digital conduct, and commitment. By checking the box below, I acknowledge that I have received, reviewed, and discussed this separate Code of Conduct document with my child, and agree that my child will adhere to all its standards. I understand that failure to comply may result in removal from the project team.*

I have read the Eyden Studio™ Student Volunteer Code of Conduct (separate document) and agree that my child will adhere to these standards.

Media and Photo Release
I grant or deny permission to Eyden Villarreal and "Eyden Studio™" to use my child's likeness, name, image, and voice (including photographs, video, and quotes) in project documentation, public relations, promotional materials, online portfolios, and social media platforms related to "Eyden Studio™" projects.*
