Thank you for your interest in enrolling your child in the AHLAN After-School Care (AASC) program by HAYBA for the year 2026.

We are pleased to share that the program will be conducted at two mosques across Singapore:
1. Al-Istighfar Mosque - East (Starts on 19th January 2026)
2. An-Nur Mosque - North (Starts on 2nd February 2026)
Kindly complete all required sections of this form accurately to facilitate a smooth registration process.

Payment Information
Please refer carefully to the payment details provided at the end of the form. Do note that enrolment is only confirmed once payment has been made and successfully verified. Seats will be allocated strictly on a first-come, first-served basis upon payment confirmation.

Sibling Discount
For families registering more than one child, a 10% program fee discount will be applied to the subsequent sibling(s). We kindly request that a separate form submission be completed for each child.

Transport Arrangement
If you require school transport, a separate form for subscription and payment will be sent to you after your registration is confirmed.

Upon successful submission of the form and verification of payment, our administrative team will add you to the relevant AASC branch WhatsApp group. All program updates and important announcements will be communicated through this channel.
