Spray Tanning Waiver & Client Consent Form

Thank you for choosing our spray tanning services. Please read the following information carefully. By signing this form, you acknowledge and accept the terms, risks, and guidelines associated with receiving a spray tan.

1. Use of Dihydroxyacetone (DHA)

I understand that the active ingredient in spray tanning solutions is Dihydroxyacetone (DHA), a colorless sugar that interacts with the amino acids in the outermost layer of the skin to produce a temporary bronzed appearance. DHA has been approved for external use; however, it should not be inhaled or come into contact with the eyes, lips, or mucous membranes.

2. Duration of Results

I understand that a spray tan typically lasts 5–10 days, depending on individual skin type, lifestyle, and aftercare routines. Proper pre- and post-tan care is essential for achieving optimal and even results.

3. Staining of Fabrics & Surfaces

I understand that the spray tanning solution may rub off or transfer onto clothing, bedding, towels, furniture, or other fabrics. While most staining washes out, some fabrics—particularly light colors, silk, nylon, or synthetic blends—may retain discoloration. I agree that the business is not responsible for any staining or damage to personal property.

4. Proper Preparation & Aftercare

I acknowledge that failing to follow recommended guidelines—such as exfoliating prior to the appointment, avoiding lotions or oils before the session, or exposing the skin to water or sweat immediately afterward—may result in uneven color, patchiness, rapid fading, or suboptimal results. I understand that results may vary and are not guaranteed.

5. Allergies & Sensitivities

I certify that I do not have any known allergies or sensitivities to spray tan ingredients, including DHA, Erythrulose, bronzers, essential oils, or cosmetic preservatives.

If I am unsure, I agree to request and complete a patch test at least 24 hours prior to a full application. I understand that the business is not responsible for allergic reactions or skin sensitivities.

6. Special Events & Trials

I understand that spray tanning results vary by individual. For major events—such as weddings, photoshoots, proms, or vacations—I acknowledge that it is strongly recommended to have a trial spray tan at least 2–4 weeks prior to the event. I accept that the business is not responsible if the result differs from expectations without a prior trial session.

7. Sun Exposure & UV Protection

I understand that a spray tan does NOT provide protection from the sun. DHA and cosmetic bronzers do not contain sunscreen and do not protect against UV radiation. I agree to use appropriate SPF when outdoors.

8. Health Disclosure

I confirm that I do not have any conditions that may interfere with the spray tanning process, including open wounds, contagious skin conditions, eczema/psoriasis flare-ups, or respiratory issues that could be aggravated by airborne mist. I agree to notify the technician of any concerns prior to my appointment.

9. Social Media & Photography Release

I understand that photographs and/or videos may be taken before, during, or after my spray tanning session for purposes including, but not limited to, education, marketing, social media, website content, and promotional materials.

I authorize Tans by Lauren to use these images and/or videos that may include my likeness, skin tone results, or body (excluding my face unless separately agreed upon), without compensation, in any media format now or in the future.

I understand that my name and other identifying information will not be used unless I give separate written permission.

I understand that I may revoke this consent at any time by providing written notice, and that revocation will apply only to future use.

9. Liability Release

By signing below, I release the business and its employees from any and all liability, including but not limited to:

• allergic reactions

• staining of clothing or belongings

• undesired cosmetic results

• complications due to undisclosed health conditions or improper aftercare

• Any additional complications as a result of this spray tan

I understand that results vary and no specific outcome is guaranteed.

If a client is under the age of 18 years old, he/she is required to have a parent (or guardian) signature agreeing that the minor is allowed to be spray tanned by Lauren Burger at Tans by Lauren. If the client is a minor, it is required that the client wear some form of undergarment  during the spray tan. The parent (or guardian) must be aware the technician may need to touch the client with a professional tanning solution paint brush, glove, or sunless applicator mitt at any time during the tanning process. For this reason, we require at least one adult over the age of 18 years old (with identification) to be present when a minor/group of minors are being spray tanned. This agreement must be signed within a week of the tanning appointment.

I certify that I am the parent or legal guardian of the minor listed below and that I have full legal authority to consent to cosmetic services on the minor’s behalf.

I understand that spray tanning is a cosmetic service involving the topical application of sunless tanning solutions containing DHA (dihydroxyacetone) and other ingredients. I acknowledge that the minor’s skin may react differently than adult skin and that results, longevity, and appearance may vary.

I confirm that all known medical conditions, skin sensitivities, allergies, medications, recent treatments (including Accutane, Retin-A, chemical peels, waxing, laser treatments, or exfoliation), and prior reactions have been fully disclosed. I understand that failure to disclose this information may increase the risk of adverse reactions and I accept full responsibility for any resulting outcomes.

I acknowledge and understand that potential risks include, but are not limited to:

Skin irritation, dryness, itching, redness, or allergic reaction
Uneven color development or fading
Transfer of cosmetic color onto clothing or fabrics
Dissatisfaction with final color or tone
I understand that spray tanning should not be performed on broken skin, open wounds, rashes, infections, or sunburned areas, and I affirm that the minor does not currently present with these conditions.

I acknowledge that no guarantees have been made regarding the final outcome and that proper pre-care and after-care are essential for best results. I accept full responsibility for ensuring the minor follows all instructions provided.

I hereby consent to and authorize Tans by Lauren to perform spray tanning services on the minor named above. I agree to release, indemnify, and hold harmless Tans by Lauren, its owners, employees, contractors, and representatives from any and all claims, demands, damages, losses, or liabilities arising out of or related to the service, except in cases of gross negligence or willful misconduct.

I understand and agree that:

A parent or legal guardian must remain on the premises during the service per business policy and applicable law.
The business reserves the right to refuse service to any minor for safety, behavioral, or skin-related concerns.
Appropriate modesty garments, protective barriers, and privacy measures will be provided. 

If you are a parent or legal guardian completing this form for a minor, please sign below stating that you agree to and give consent to all information stated above. By signing this form you agree to the minor receiving a spray tan from Tans by Lauren.
