Thrive Aesthetic and Wellness Membership Contract

This Membership Agreement (“Agreement”) outlines the terms and conditions for members enrolled in the Thrive Aesthetic and Wellness Membership Program. By enrolling, you agree to the terms below.

1.1 Minimum Commitment
All memberships require a 12-month commitment with automatic monthly payments or pay in full at the time of agreement.

1.2 Cancellation Policy
By signing this Agreement, you authorize Thrive Aesthetic and Wellness to charge your account monthly for 12 months at the agreed membership tier rate or, if you prefer, you may choose to pay the full 12-month membership amount upfront.

• Monthly Autopay Option: Your account will be charged automatically each month for 12 consecutive months.
  
  
• Pay-in-Full Option: If you elect to pay the entire 12-month membership cost upfront, you will receive the additional membership perks outlined in Section 3, including the ability to schedule services back-to-back and an extra VIP service. This payment is non-refundable once processed.
  
  

1.3 Early Cancellation
If you cancel before the end of your 12-month term, you agree to pay:

• The full retail value of any services already received, minus amounts already paid into the membership; plus
  
  
• A $150 early cancellation fee.

2.1 Non-Transferable
Memberships are for individual use only and cannot be shared, transferred, or assigned.

2.2 Treatment Expiry
All treatments included with your membership must be used within your 12-month contract. Unused treatments do not roll over and will be forfeited at the end of the term.

2.3 No Cash Value
Membership payments and benefits hold no cash value and cannot be redeemed for cash, refunds, or credits

Membership benefits vary by tier:

• Refresh Membership: $166/month or $2,000/year – Get 10% off product and services, save $50 per syringe of filler, choose 4 services from the VIP list per year.

• Monthly Payment Option: Members may receive 1 VIP service every 3 months.
  
  
• Pay-in-Full Option: Members may receive VIP services back-to-back and will also receive 1 additional VIP service during the membership year.
  
  

• Revive Membership: $250/month or $3,000/year – Get 10% off product and services, get $1 off/unit on tox, save $50 per syringe of filler, choose 6 services from the VIP list per year.

• Monthly Payment Option: Members may receive 1 VIP service every 2 months.
  
  
• Pay-in-Full Option: Members may receive VIP services back-to-back and will also receive 1 additional VIP service during the membership year.
  
  

• Thrive Elite Membership: $350/month or $4,200/year – Get 15% off product and services, get $1 off/unit on tox, save $50 per syringe of filler, choose 9 services from the VIP list per year

• Monthly Payment Option: Members may receive 1 VIP service every 1 month until all 9 services have been received.
  
  
• Pay-in-Full Option: Members may receive VIP services back-to-back and will also receive 1 additional VIP service during the membership year.

Important: Back-to-back services are only permitted if the membership is paid in full upfront.

Financing: If you choose to use third-party financing through Cherry or PatientFi, an additional 5% financing fee will be applied and collected at the time of payment.   

You authorize Thrive Aesthetic and Wellness to automatically charge the account provided for membership fees. If a payment is declined:

• You will be notified and must provide updated payment information within 7 days.
  
  
• A $25 late fee will be applied for payments not resolved within this timeframe.
  
  
• If payment is not received within 30 days, membership benefits will be suspended until the account is brought current.

By enrolling in this membership, you acknowledge:

• Treatments are elective and results vary per individual.
  
  
• You are responsible for providing accurate medical history and informing staff of any changes.
  
  
• Thrive Aesthetic and Wellness and its providers are not liable for complications resulting from undisclosed medical conditions or failure to follow aftercare instructions.

Thrive Aesthetic and Wellness reserves the right to modify or update membership terms with 30 days’ written notice. Continued use of the membership constitutes acceptance of updated terms.

• Acknowledge the cancellation, usage, payment, and liability policies outlined above.
