I. To be a client of Max Wellness by Krystal, I must be willing to do what is necessary for my health within reasonable limits. If I am not willing to work on my health/diet/lifestyle, then this program may not be for me. I understand that it is expected of me to keep scheduled appointments to ensure maximum progress.
II. I understand that I am here to learn about nutrition and better health practices. I will be offered information about food supplements and herbs as a guide to general good health. This consultation is offered as a personal ministry.
III. I understand that we may recommend support higher dose of nutritional supplementation than manufacturer recommendations. I understand that I can adhere to product label recommendations at my discretion. Practitioner-grade products sold online (such as on Amazon) may be counterfeit, expired, or unsafe.I will purchase recommended supplements from Max Wellness by Krystal.
IV. I fully understand that Krystal Maxwell and those who counsel me are not medical doctors, and I am not here for medical diagnostic purposes or treatment procedures. I am not, on this visit or any subsequent visit, an agent for federal, state, or local agencies or on a mission of entrapment or investigation.
V. The services performed here are at all times restricted to consultation on nutritional matters intended for the maintenance of the best possible state of natural health, and do not involve the diagnosing,treatment or prescribing of remedies for disease. I have read and understand the foregoing. This
permission form applies to subsequent visits and consultations.
CANCELLATION POLICY: All visits are pre-paid. To reschedule, you must contact us within 24 hours at maxwellnessbykrystal@gmail.com
. Refunds are not issued but will be transferred to your next available time slot. If you do not answer at your appointment time, we will call once more within 5–10 minutes. Missed calls will be marked as a no-show and require booking a new appointment.
