Please note that this form is only to fill out the REQUIRED paperwork for you or your contestant. This form must be filled out by everyone either here or mailed in a physical copy from the website. This form will not ask you to make a payment as we have other links/forms for that. 

In your confirmation email you will have a button/link to go back and edit your form if needed. It is advised to download your paperwork after submitting it for you to look back on what you put in your bio (10+ for interview) as well as see what all you entered in.

DEADLINE FOR ENTRY FORM PAGES 1 & 2, PHOTOS, AND FEES IS JUNE 14, 2026

Cash, check, or cashier's checks are only sent via mail. Personal checks must be postmarked by 6/14/26 and written to AmeriFest LLC. Enter online, by mail to AmeriFest LLC P.O. Box 1174 Richmond, KY 40475, or in person at our studio location.

In signing this entry form I have read and understand the contents of this packet and agree with all rules and regulations of AmeriFest LLC. I understand there are no refunds, transfers, or forwarding of fees paid, this includes the "Claim Your Spot" payment. AmeriFest pageants, host venues, judges, staff, or other affiliated parties are not responsible for injuries during, traveling to or from the event. AmeriFest is not responsible for lost or stolen items. We reserve the right to decline any advertising we see not suitable for our audience. We reserve the right to combine age divisions with two or less contestants. We reserve the right to substitute any gifts or prizes for those of equal or greater value. Poor sportsmanship by a contestant or family member will result in disqualification of titles and/or any prize or competing within the AmeriFest systems in the future. We reserve the right to deny any future re-entry into the pageant based on poor sportsmanship. Judges decisions are final and will not be disputed. All results are tabulated electronically. All information above is correct and true. If age of contestant is questioned at the pageant and you do not have a legal, certified copy of age verification, such as birth certificate, your title could be in jeopardy at the event. Lying about a legal age and accepting gifts under that pretense is considered theft by deception. Anyone over the age of 16 may not have anyone in the dressing area with them. Anyone 16 & under may have one female present.

Family Discount Program: Granted for those in same household / par- ents only. Or parent / child that is competing. The discount is 1/2 off of the supreme package for each qualifying contestants total entry. Does not apply when the ad program is also being used.

__X__REGISTRATION FEE $100 This fee is free by winning your state title or at-large title.

__X__SUPREME PACKAGE $395 Includes Grand Supreme, Age Division Queen’s Court judged based on evening gown competition (plus interview for 10 & up). FREE by selling 395 chances or selling 3 ad pages! Ad page discount cannot be used with Ky Grand Supreme pageant cash.

NATIONAL PROGRAM BOOK $40 Must be reserved by paperwork deadline. The order cannot be canceled if the reservation is selected. If you have earned one free through ads, please still mark above to ensure a reservation. All contestants will appear in the program book in age divisions. Headshot must be submitted by paperwork deadline or AmeriFest logo will appear with the name.

OFFICIAL NATIONAL SHIRT $30 plus fees Available to everyone! Required for ages 7 and up. Ages 7-9 will wear in opening parade of contestants with your choice of white bottom. 10 and up will wear rehearsal and group photos. BEAUTY & PERSONALITY SUPREME $50 4th & 5th highest overall in your age session. Included in Supreme Package for STAR Contestants. Awarded per session.

SIDE AWARDS $50 Most Beautiful/Handsome, Best Dressed, Best Hair, Personality, Rising Star. Awarded per division.

COVER MODEL $50 Choice of up to 3 head shots. Any size photo up to 8x10. No AI photos. Winner will receive an award on stage plus your photo in the program. Awarded per session. Judged before the pageant– mail in or submit online.

CENTERFOLD $50 Choice of up to 3 full body shots. 8x10 size preferred. No AI photos. Winner will receive an award on stage plus your photo in the program. Awarded per session. Judged before pageant– mail in or submit online.

THEME PHOTO $50 Choice of up to 3 themed photos of contestant’s choice. 8x10 size preferred. No AI photos. Winner will receive an award on stage plus your photo in the program. Awarded per session. Judged before the pageant– mail in or submit online.

PHOTOGENIC $50 Up to 3 photos of any size up to 8x10. No AI photos. Awarded per age division. Judged before the pageant– mail in or submit online.

EXTRA PHOTOGENIC ENTRIES $50 Up to 3 extra photos of any size up to 8x10, in addition to any entered in Photogenic. No AI photos. Awarded per age division. Judged before the pageant– mail in or submit online.

RUNWAY WEAR $50 Rock the runway in any color/ style (must be appropriate) outfit. Be creative and have fun! Awarded per age division.

FESTIVAL WEAR $50 Model an outfit on stage that best represents your state! Be creative and have fun! Awarded per age division. Bring an index card with explanation of outfit turned in during line up.

FITNESS WEAR $50 Model your fitness outfit on stage! Be creative and have fun! Awarded per age division. For ages 4+ (Little) only. Must be age appropriate.

SWIM WEAR $50 Model your swim wear on stage! Awarded per age division. For Teen+ only. Must be age appropriate.

TALENT $50 Up to 90 second talent. Microphone provided.NO other instruments provided by AmeriFest nor hotel staff. Music will be turned in at rehearsal (CD or jump drive). 2 per session & one overall “Entertainer of the Year” for ages 4-12, STAR, and one for 13+.

A.L.I.C.E. COMMUNITY SERVICE Free to enter. 4-9
will submit an essay online or at registration on “What Community Service Means to Me.” Must also list service activities within the past 2 years. Can be 2 sided, but must be 1 page. Ages 10 and up will conduct a speech/ interview with judges after regular interview. Awarded per session.

HUMANITARIAN Free to enter. Awarded to the contestant collecting the most toys, pet supplies, food pantry items, books, or stuffed animals for children before pageant weekend. Forms available on website and must be signed by an authorized personnel. Awarded per session.

AmeriFest Supermodel

In order to be eligible for this title, you must compete in the following optional categories:

Mister, Wee, and Star:
Runway Wear and Festival Wear.

Little, Junior, & Preteen:
Runway Wear, Festival Wear, and Fitness Wear

Teen, Miss, Ms, Mrs, Woman, Elegant, & Modern:
Runway Wear, Festival Wear, and Fitness Wear OR Swim Wear

The highest-scoring contestant in each session will go to supreme row and be crowned this title!

A fun way to earn your fees paid and lots of awesome prizes is to sell good luck pages or business ads for the official program book! The sponsor pages may be sold to family members, friends, businesses or anyone who wants to sponsor you! They may be business advertisements or pages to say "good luck". All ads must be mailed to PO Box 1174 Richmond, KY 40476 or online at www.AmeriFestPageants.com. Ads must be sent as you would like them to appear in the pro- gram book and must be ready to be scanned (if submitted through mail We do not design ads, they must be print-ready. Absolutely no refunds on any ads. No staples, laminations, tape, or foam board. If you are uploading online, you do not need ad receipt attached to the ad submitted online. The online deadline is 06/14/26 at midnight EST.

In addition to the Pageant Ambassador Award, the ONE contestant with the most ad pages will win the title, rhinestone bling blanket, custom national crown valued at $150, a full page photo in the program book, diamond accent jewelry, 2 free tickets to the Gala, $200 travel cash, taxes paid for the trip above, and upgraded supreme sash!
