3. No Diagnosis or Treatment

4. Not a Substitute for Medical Care
I agree to seek a licensed medical professional for any symptoms, concerns, or emergencies.
5. Voluntary Participation
My involvement is completely voluntary. I may stop at any time.
6. Frequency Harmonization
I consent to receiving:
Non-invasive frequency balancing
Energetic harmonization
Headphone-based frequency delivery
7. Individual Results Vary
I understand that no specific results are guaranteed.
By signing this form, I agree to the following:
1. Assumption of Risk
I understand that energetic wellness services may influence emotional, physical, or energetic states.
I accept all risks associated with participating.
2. Release of Liability
I release and hold harmless Frequency Goddesses and all affiliates from any liability related to:
Emotional or energetic responses
Choices made after receiving information
Interpretation of energetic findings
Any real or perceived effects of services
3. No Guarantees
I understand no outcomes, improvements, or healing results are promised or implied.
4. Personal Responsibility
I am fully responsible for my wellness decisions.
Frequency Goddesses voluntarily maintains high-level privacy standards.

1. Non-Licensed Services
These services are not licensed by the State of North Carolina.
2. No Practice of Medicine
The practitioner does not practice medicine, psychology, therapy, or any licensed healing arts.
3. Continued Medical Care
I am encouraged to maintain regular medical care with licensed healthcare providers in North Carolina.
4. Understanding of Limitations
I understand these services are complementary, non-medical, and informational only.

This Hold Harmless Agreement (“Agreement”) is made between the named party on this form and Angel Collective, LLC. dba Frequency Goddesses, its owner, employees, contractors, affiliates, and representatives (“Practitioner”).
1. Nature of Services
The Client understands and acknowledges that:
Angel Collective, LLC. dba Frequency Goddesses provides non-medical, energetic wellness services, including Bioscan energetic assessments, frequency harmonization sessions, and educational wellness guidance.
These services are based on energy and frequency principles, and are not medical, diagnostic, therapeutic, psychological, or clinical in nature.
The Practitioner is not a medical doctor, licensed clinician, or healthcare provider.
2. No Medical Claims
The Client acknowledges that:
The services offered do not diagnose, treat, prevent, or cure any medical or psychological condition.
No statements made during sessions or in written materials have been evaluated by the FDA or any medical authority.
The Client is fully responsible for obtaining appropriate medical care from a licensed healthcare professional.
3. Assumption of Risk
The Client voluntarily assumes full responsibility for any and all risks associated with participating in:
Bioscan energetic assessments
Frequency harmonization
Energetic or emotional responses that may occur
Decisions made based on information received
The Client understands that results vary and that no guarantees are offered or implied.
4. Hold Harmless Provision
The Client agrees to hold harmless, indemnify, and release Angel Collective, LLC. dba Frequency Goddesses and all associated parties from:
Any and all claims,
Damages,
Liabilities,
Losses,
Injuries,
Emotional or physical responses,
Consequences of actions taken based on session information
whether foreseen or unforeseen, arising out of the Client’s participation in any service provided.
This includes, but is not limited to:
Financial loss
Emotional distress
Perceived or actual energetic effects
Misinterpretation of information provided
Actions taken or not taken by the Client
5. No Guarantees
The Practitioner makes no promises, warranties, or guarantees regarding:
Outcomes of Bioscan readings
Energy harmonization results
Healing effects
Symptom changes
Emotional, physical, or spiritual improvements
The Client understands all services are experimental, informational, and complementary in nature.
6. Client Responsibility
The Client agrees to:
Seek immediate medical attention for any physical or mental health concerns.
Continue routine medical care.
Use their own judgment when applying any information received.
Notify the Practitioner of any discomfort during a session.
7. North Carolina Compliance
This Agreement complies with the laws of the State of North Carolina, and acknowledges that:

The Practitioner does not practice medicine, psychology, therapy, or any licensed healing arts.
The Client has been encouraged to consult licensed NC healthcare providers for all medical needs.
8. Entire Agreement
This Agreement represents the complete understanding between the parties and supersedes all previous discussions or agreements relating to liability.
9. Signature & Acceptance
