2026 Midwest All Star Championships - Team Colorado

January 9-11, 2026 - Des Moines, IA
DEADLINE: DECEMBER 15th MONDAY

Steps for confirming your place on Team Colorado:
1. Complete the All Stars Confirmation below
2. Fill out the Code of Conduct, Medical form, Photo Release form (all forms were attached to the announcement email and are available on the CSI All Stars website (LINK HERE) and submit via below in form:
3. After Form is recieved an email invoice from Quickbooks will be sent to Parent #1. Then SUBMIT PAYMENT via link emailed from Quickbooks Payments (ACH and CC accepted).
Steps for declining your team acceptance:
1. Choose, no I will not be attending and then hit submit (please do not fill out any other items)
Costs:

· 11 & Over Swimmers   - $840.00

·         10 & Under Swimmers - $290.00

Chaperones:

If you are interested in chaperoning, please head to the website for all stars for application
