Liability Waiver and Release

By signing this form, I acknowledge and agree to the following terms as a participant (or parent/guardian of a participant) in Waveffect and the Run It programs, events, and activities:

1. Assumption of Risk:
I understand that participation in basketball and related activities involves inherent risks, including but not limited to physical injury, illness, or accidents. I voluntarily assume all such risks, whether anticipated or unanticipated.

2. Release of Liability:
I agree to release and hold harmless Waveffect and Run It, their coaches, staff, volunteers, affiliates, sponsors, and facilities from any and all claims, demands, causes of action, or liability for injuries, damages, or losses arising from participation in activities associated with the league or academy.

3. Medical Authorization:
In the event of an emergency, I authorize Waveffect and its representatives to obtain medical treatment deemed necessary for the participant. I agree to assume responsibility for all associated medical costs.

Media Release Consent

I hereby grant Waveffect and Run It permission to photograph, film, or record the participant during events, practices, and related activities. I understand that these images and recordings may be used for promotional materials, including but not limited to:

Social media platforms

Website content

Flyers, brochures, and other marketing materials

Newsletters and press releases

I waive any right to inspect or approve the final materials and release Waveffect and Run It from any claims related to the use of these materials.

Payment Policy
RUN IT PROGRAM — PAYMENT POLICY (For Application)

PAYMENT INFORMATION (READ BEFORE SUBMITTING APPLICATION)

Submitting this application does not require payment. The RUN IT Program accepts only 10 athletes, and all applications are reviewed for fit, commitment, and competitive level.

If your athlete is selected:

✔️ You will receive an official acceptance message
✔️ You will be given 48 hours to complete payment
✔️ Your spot is ONLY secured once payment is completed

If payment is not submitted within 48 hours, your spot will be released to the next athlete on the waitlist.

PROGRAM COST:

$200 total for the full one-month Run It program

Includes all training sessions + media content + docu-series access

NO PAYMENT IS COLLECTED TODAY.

Payment is only required after acceptance into the program.

By submitting this application, you acknowledge that:
