Do you have a budget for your tattoo? If yes, please list the amount. (My prices are by piece, with larger pieces being hourly: $150hr, Day rate $900 (up to 6 hours), and my minimum $100. I'm always happy to do my best to work within any budget! We could break the piece into sessions or parts if needed)

How far are you traveling?

I understand that there is a $50 non refundable deposit to book an appointment. If I need to reschedule my appointment, I understand that less than 48 hours notice will result in the forfeiture of my deposit.

I understand that there is a 15 minute grace period, and showing up later than 15 minutes past the start of my appointment time will lead to the cancellation of my appointment and forfeiture of my deposit.

I understand that design previews must be scheduled in an in person consult. (I do not send design previews through messages) Any changes to your idea that is different than what was accepted may result in a decline of the project or a reschedule to allow me more time to properly design.
