Winter Session COURT Connections Registration
Thank you for your interest in participating in one of our COURT Connections classes at Premier Tennis Center! These classes are designed for players looking for more exposure to tennis. Participants will enhance their tennis game while also completing activities in the be nice. curriculum. After completion of this registration form, Caroline Phillips will email you to confirm your spot is secured in this class. If you are looking to apply for a scholarship into one of Premier Tennis Center's other classes, please email cphillips@wmctpremier.com for the correct form. At this time, we are unable to provide scholarships to students who are also participating in COURT programs. Players who participate in COURT programs can also participate in paid programs; however, they cannot receive additional scholarships.

Participation Waiver: By submitting this form, the applicant acknowledges that participation in a sport or physical activity may result in accident or injury and assume the risk connected with such participation. The student applying to participate in this program is in good health and suffers no physical impairment that would limit their use of the club's fitness facilities, or any physical impairments have been disclosed to the program staff. The applicant acknowledges that the club staff has not and will not render any medical services including medical diagnosis of the participant's physical condition.
