Artist Application Form - MEMBERSHIP RENEWAL 2026

DEADLINE DECEMBER 31, 2025. Please complete this MEMBERSHIP APPLICATION FORM, read the attached MEMBERSHIP TERMS, and click the agreement box. Required fields are marked with an asterisk (*).  You can save your unfinished form at any time after providing your email. After saving, you will receive an email containing a link that allows you to return and continue with your form.

    What you'll need

    • A very brief statement about your art -- just 2 or 3 sentences.
    • Payment info: have your Credit or Debit card or PayPal account handy.

    You can now save your unfinished form at any time. After saving, you will receive an email containing a link that allows you to return and continue with your form.

    PLEASE NOTE: If you wish to save your form and continue later,
    you do not need a JotForm account.

    Please note the option “Skip Create an Account” in fine print at the bottom. If you see this dialogue box, you can click on that option to save your form.

     

    Also: on a desktop or tablet, the "save" icon should be visible at the bottom of the screen.

    On a phone, the "save" icon will appear at the top of the screen if you click on the page number at the bottom, as shown in the image below left. As shown in the image below right, clicking on the page number will display the "save" icon and a clickable menu of all the form pages.

    Almost Done. The next section will offer payment links to PayPal.

      On the next page, the Annual Membership fee should already appear as "1" in the shopping cart at upper right.  Please click on the NEXT arrow to continue with payment.


    On the next page, you will see a yellow PayPal button and a black credit/debit card button. If you don't wish to use PayPal, click on the black button.

     

    After that, you'll see a page like the image below. The "Pay Later" option will prompt you to create a PayPal account. If you click the "Pay Now" button on the left, you'll continue without having to do that.

     

    After filling in your payment information, you will be invited to "Save info and create your PayPal account", as shown below. You can make sure the toggle is not activated and proceed to the "Pay Now" button near the bottom of the page. Below that you'll see a prompt to "Log In" -- that's only for PayPal account holders. If you have filled in your own payment information, you can simply click on "Pay Now".

    There is no need to Log In or create a PayPal account.

    Once your completed Application is sent, you'll receive an e-mail confirmation that will include your completed form.
