    2026-2028 Call for Proposals
  • To ensure a complete and successful submission, please follow the steps below.

    Steps to Complete Your Application

    1. Read the Reaching Home Application Guide. Review all eligibility criteria, mandatory requirements, and funding priorities before beginning your application. The Guide explains what activities can be funded and how proposals will be assessed. 
    2. Develop your project idea. Ensure your proposed project aligns with Reaching Home outcomes and the priorities identified in the Community Action Plan (CAP).
    3. Gather organizational information and documents. Confirm  policy, insurance, finance, or governance details that may support your responses.
    4. Prepare your budget. Download the Reaching Home Budget Template, complete all required fields and ensure all costs are eligible and clearly justified. Save your file as: Organization Name_RH Budget 2026-28.xlsx.
    5. Complete this Reaching Home Funding Application. Answer all questions thoroughly. Upload your completed budget in Section 5.1 of the Application.
    6. Review your application. Confirm that all questions are answered and the completed Budget Template is attached.
    7. Submit your application by clicking SUBMIT at the end of this Form before the deadline. All applications must be submitted no later than Monday January 26, 2026 at 11:59PM YT. Submissions or edits will not be accepted after this time.

    Saving, Returning and Editing Your Application

    • Save and Continue Later - You may save your progress at any time by clicking SAVE at the bottom of the form and entering your email address to receive a personalized return link.
    • Edit Before Submission - You may reopen your saved form and make changes as often as needed prior to submitting.
    • Edit After Submission (until deadline) - After you submit, you will receive a confirmation email containing an "Edit Submission" link. You may use this link to make updates or corrections until the deadline, after which editing will be disabled.

    Guidance on Response Length and Content Quality

    There are no strict word limits, but we ask that repsonses remain clear, concise and focused. Shorter questions may require only brief responses. More detailed questions should generally remain under 250-300 words to ensure readability and clarity. 

    Please ensure your responses are thoughtful, informative, and directly connected to your proposed project.

  • Briefly describe your strengths, experience, and expertise supporting people experiencing homelessness - especially those with chronic or complex needs. How will these strengths support the successful delivery of your proposed project?

    For returning proponents: Include any additional homelessness-related work beyond your current Reaching Home-funded activities. 

    For new proponents: Include your organization's experience delivering homelessness-related services or similar social support programs. If you are new to this work, outline the foundations, partnerships, or internal practices that prepare you to deliver a housing-focused model.

     

  • Section 2.0: Organizational Capacity and Mandatory Requirements

  • Proponents must confirm their readiness to meet the mandatory requirements outlined in the Reaching Home Application Guide. If you have questions about these requirements, or if you are unsure about your capacity to meet them, please let us know. The YAPC Reaching Home Team is available to discuss what support may help your organization meet these expectations.

  • Reaching Home-funded organizations must be able to conduct activities in the community that support participants with their housing. Can your staff perform the following:

    • Quarterly activity and financial reporting
    • General ledger submission
    • Project monitoring participation (including onsite visits and provision of source documents)

  • Proponents are strongly encouraged to review the Community Action Plan to ensure your project aligns with local priorities.

    Please note the Community Action Plan is in its final stages of completion and will be made available and put on YAPC's website at the end of December.
