As a volunteer of Methodist Children's Homes of MS, I agree to abide by the policies and procedures. I understand I will be volunteering at my own risk and the organization and its employees and affiliates cannot assume any responsibility for any liability for any accident, injury or health problem which may arise from any volunteer work I perform for the organization. I agree that all the work I do is on a volunteer basis and I am not eligible to receive any monetary payment or reward.

VOLUNTEER CONFIDENTIALTY STATEMENT

As a volunteer of Methodist Children's Homes, I understand that I am required to protect confidential information furnished to me concerning the youth in the group homes, foster home and/or their families. This includes the youth's name or any related identifying information whether told or overheard regarding any resident. I understand that any identifying information, pictures, or documents obtained may not be released to anyone or any agency nor should it be discussed with anyone other than the staff of Methodist Children's Homes, unless written consent from the legal guardian is obtained and verified. I further realize that this confidentiality agreement is not applicable when it comes to reporting any suspicion, allegation, or first-hand knowledge of abuse/neglect involving a youth in the group home or foster home. By signing below, I also agree to report any such suspicion, allegation or first-hand knowledge of abuse or neglect of a resident in the care of Methodist Children's Homes, immediately to Management and CPS at 1-800-222-8000. I further agree to testify on behalf of the resident, if asked to do so, in regards to any report that you make regarding suspected abuse or neglect.
