Application For Financial Assistance
Dear Players and Parents,

The Ohio Girls Golf Foundation accepts Financial Assistance applications on a quarterly basis. Award amounts are determined by the availability of funds and the total number of applications received.

To apply, please complete the Financial Assistance Questionnaire and Tournament Expense Report below. Be sure to upload copies of receipts for any incurred expenses and provide confirmation of tournament participation.

If you require funds in advance of a tournament, please contact Lynne Plaisance at (440) 479-9579.

Deadlines to Submit Quarterly Applications:

First Quarter - March 31
Second Quarter - June 30
Third Quarter - September 30
Fourth Quarter - December 31

Notes:
I. The Foundation will abide by the USGA Rules of Amateur Status including those of expenses and age restrictions.

II. Each award of financial assistance to tournaments will be based on availability of funds in addition to applicants' qualifications. Some tournaments have an annually predetermined amount of financial assistance to be awarded which may be adjusted for proximity of the event to hte applicant's residence.

III. The amount of financial assistance may vary from year to year based on contributions to the Founation and submission of applications and in no way does the Foundation guarantee any candidate, regardless of qualifications, tht they will recieve any form of financial assistance.

IV. Financial Assistance will be awarded based on determination of a financial assistance committee. Applications which are not completed by the appropriate date will not be considered.

V. The Foundation does not discriminate against any applicant because of race, religion, color, sex, national origin, or handicap.

VI. The Foundation will require receipts for the reasonable expenses incurred by financial assistance receipients while attending the designated golf tournament.

VII. Financial Assistance will be distribue after the receipt of required documentation of participation and expenses. Completed applications will be accepted quarterly.
