Parental Consent and Liability Waiver

In consideration of the 2025/26 Cowichan Capitals allowing me to participate, I agree and understand the following:
Cowichan Capitals reserves the right to reschedule or regroup players according to enrolment and availability. Cowichan Capitals  and Cowichan Community Arena employees will not be liable or held responsible for any accidents or loss however caused and agreed to release all mentioned parties from all claims and damages. I further verify that my child, my ward, or I have no medical problems and is in good physical health and the above-mentioned parties will not be held responsible for any medical, dental or insurance claims resulting from injury.

Media Release Agreement
I, the undersigned parent or legal guardian of the participating youth, hereby grant permission to the Cowichan Capitals Junior Hockey Team to photograph, film, or otherwise capture media of my child during involvement in any Cowichan Capitals youth programs.

I understand and agree that: - Media may be used for promotional, educational, and public relations purposes. - Media may appear in printed materials, online platforms, social media, and other communications produced by the Cowichan Capitals organization. - My child’s name may be used unless otherwise indicated below. - No financial compensation will be provided for the use of these materials.

If I wish to revoke this permission, I understand that I must provide written notice to the organization.
