CLASS SELECTION

Please see below and select your classes you would like to enrol in. Please refer to our weekly program before selecting your classes. Classes on our program indicate the appropriate age an athlete should be to join each class.

PAYMENT OPTIONS & REGISTRATION FEES
 

The Enrolment Registration Fee is a mandatory, yearly fee that must be paid upon submission of this enrolment form.

Harriman HEARTS Member Fee (Ages 2–5): $30
Harriman ACTIVE Recreational Member Fee (Ages 5–12): $30
Harriman ACTIVE Recreational Member Fee (Ages 13+): $40
HSQUAD The Competition Squad Member Fee (All Ages): $250
EPS School Squad Member Fee: $100
 

Important Notes:
 

This is a yearly fee, paid once per calendar year.
No refunds will be given after payment is received.
Participants will not be allowed to join class without this form and the required payment being submitted.

Assumption of risk
I am the participant (over age of 18) or parent, carer of participant (under the age of 18) named on this document. I recognise that minor and or major injuries can include strains, sprains, broken bones, or other more serious injury in dance, acrobatic and fitness activities. I clarify that I deem my child competent and fit to participate in all activities associated with the classes/disciplines I have selected for their participation, and I accept ALL risks associated with their participation.

Release of liability
In consideration of myself, or my child(ren)s participation, I hereby for myself and my child(ren), and our respective heirs and successors, promise NOT to sue and forever release and discharge Harriman DSAF, it's coaches, directors, employee and volunteers, from all liability resulting from damages or injuries incurred as a result of participation in Harriman DSAF activities, programs and competitions, including those resulting from acts of negligence without permission.

Medical Emergencies
In case of an emergency, I hereby give consent to certified Harriman DSAF coaches and/or health professionals to administer emergency first aid to the athlete and/or obtain medical and/or ambulance assistance as required. I agree to pay any costs incurred in relation to this.

Charges and class schedule changes
Harriman DSAF reserves the rights to increase fees and charges with 30 days notice. Class times and days are not likely to change although HDSAF reserves the rights to move classes for reasons of large numbers in a class and/or not the minimum number in a class for that class to run. Changes will normally come into effect commencement of the following term.

Enrolment and Payment Conditions
 

 

Enrolment and Due Dates
 

Enrolment officially commences on the date stated on the enrolment agreement.
The Registration Fee must be paid before the beginning of the participant’s first enrolled lesson.
Class Fees are due within the first two (2) weeks of the commencement of each term.
Invoices will be sent to your nominated email address once the enrolment form has been successfully submitted.
Fees are calculated based on the classes nominated and are charged regardless of attendance.
 

Family Discount Policy
 

A discounted fee of 10% only applies to families enrolling multiple members.
The 10% discount will be applied to the fee payable of the second enrolled member and any additional family members enrolled thereafter. The first enrolled member receives no discount.
Discount Exclusions:

Please note that the standard family discount is not applicable to the following additional training sessions: Private Training (PT), EPS School Squad, National HSQUAD training, or any holiday programs offered during the year.
 

Training Schedule and Public Holidays
 

Our normal weekly programmed classes run in four terms, in alignment with public school terms, and do not operate during school holidays.
Classes will still operate on Public Holidays for most of the year. This continued training is especially important for competition-enrolled members. Clarification regarding Public Holiday operation will be sent out prior to the day.
If classes are offered during school holidays, on Sundays, and/or on extra scheduled training days, a separate invoice will be sent out prior to these lesson dates.
 

HSQUAD Payment Structure
 

HSQUAD members commit to an annual fee, which is split into four (4) equal payments.
This commitment covers the full year, and the weekly amount remains the same throughout the year during scheduled training periods.

Late Fee Policy (Strictly Enforced)
 

Payment of class fees is strictly due within the first two (2) weeks of the commencement of each term.

If your payment is not received by the stated due date, you will be issued a reminder and granted a 14-day grace periodto finalise the outstanding balance.

Failure to complete the full payment within this 14-day period will result in the immediate application of a late payment fee to your account. Consistent failure to meet payment obligations may lead to suspension from classes.

Cancellation Policy (Term Fees)
 

Cancellation of enrolment is only permitted and approved PRIOR TO THE COMMENCEMENT OF A NEW TERM.

Cancellation requires two (2) weeks' written notice and must be submitted via email to harriman.dsaf@outlook.com.

 

Critical Notice Regarding Term Fees:
 

NO CANCELLATIONS OR REFUNDS ARE PROCESSED ONCE A TERM HAS COMMENCED.

If notice is provided after the term has started, your request is considered a cancellation for the subsequent term, and you remain liable for the full current term fees.

Failure to provide the required two (2) weeks' written notice prior to the start of a new term will result in your class enrolment not being cancelled, and you will remain liable for the full term fees.

Please Note: Verbal notifications or messages to coaches are not acceptable forms of notice. The date of cancellation is effective from the date the written email is received by harriman.dsaf@outlook.com.

Refund Policy
 

All payments, including deposits, are strictly non-refundable.

Refunds are not permitted for:

Change of mind.
Missed lessons due to minor sickness, short holidays, or minor injuries.
 

Conditions for Approved Refunds
 

Refunds will only be approved in cases of certified long-term travel or serious sickness/injury that significantly prevents participation.

Long-Term Travel: Travel exceeding 28 days (4 weeks) during term dates may be considered.
Serious Sickness/Injury: Must be substantiated by a valid Doctor's or Specialist's Certificate confirming the necessity for long-term recovery.
Please Note: In most cases of minor sickness or injury, the athlete/dancer is still able to attend classes and work on other areas of their training (e.g., strength, theory, conditioning). Our coaches are always willing and able to provide modified training options in these circumstances to maintain the continuity of development.

Missed lessons
Hearts Members- Make up lessons are not guaranteed but may be available by filling in a make up lesson form on the HDSAF website. If you miss a lesson TWO make up lesson can be offered, per participant during the same term.

Active Members- Make up lessons are not guaranteed but may be available by filling in a make up lesson form on the HDSAF website. If you miss a lesson ONE make up lesson can be offered, per participant during the same term.

Please do not email through your make up option date and class, we will only accept forms submitted through our website. Make up lessons can be any class in your age group, on any day and any location. Please only attend your make up lesson with approval from our team, there may not be available spots in the class you have requested. 

HSQUAD Members- Your Competition Squad membership requires attendance for a specific number of training days per week (e.g., two days per week). You must attend the total number of days you pay for within that specific week.

Please note: We do not offer make-up lessons to be carried over to another week during the term. If you miss a paid session, you must attend one of the other scheduled sessions that same week to ensure you meet your commitment.

EPS School Squad Members- Please note that HDSAF does not offer make-up lessons for missed School Squad training sessions. Due to the condensed term structure and specific team choreography, all scheduled sessions must be attended. Fees cover the entire term, regardless of individual attendance.

Serious sickness, injury or long periods of overseas travel
Harriman DSAF will only credit accounts and/or refund term classes and charges with written notice, we prefer as much notice as possible. Supporting evidence relating to your request include medical certificates from your specialist and travel dates of more than 28 days.

Parking Policy
Ermington Studio ONLY
It is important that all parents and members park outside of the school grounds at Ermington PS. Parking available on Brush rd. Please only enter through school gate on Brush Road to access hall. Strictly no parking or driving onto the school grounds.
Both studio locations
Please make sure each child is accompanied by an older person and dropped to the door of both premises, ready for class. Please don't allow your child to leave the building and/or school unaccompanied. A coach will not allow a child to walk home by themselves.

Safety and Sickness Policy
SAFE PROCEDURES
I will agree and abide by the policies and procedures HDSAF have in place during their classes. Parents and caretakers to be outside of studios at all times and only allowed to view classes on special events and open days set by HDSAF. I will keep myself and/or my participant home if unwell at all times.

Social Media and Image Approval
 

By agreeing to this media agreement, the Parent/Guardian grants permission for Harriman DSAF to capture and use photographs or video footage of the participant during classes, performances, events, and training sessions. This media may be used for promotional and marketing purposes, including but not limited to, the studio website, social media platforms (e.g., Instagram, Facebook), and print materials. No full names or personal contact information will be published without explicit additional written consent.

Inaccurate Contact Details
I understand that if I do not provide accurate and up-to-date contact information I may miss out on important and timely information which may affect my enrolment or result in missed opportunities.

Participation
I confirm that the information set out on this form on behalf of the athlete/dancer is true and correct to the best of my knowledge and is being relied upon by HARRIMAN DSAF and its staff. I know of no reason or fact affecting the members health which would affect or preclude him/her from physical activities.
