Snow Removal Program

We are pleased to announce that our volunteer-driven Snow Removal Program is now in effect. This initiative is designed to assist seniors aged 60 and older, as well as neighbors with disabilities, by clearing snow from the front steps and the front sidewalk area directly leading up to their home.

Please note that volunteers will be dispatched only when the snow reaches a certain minimum accumulation. Additionally, snow removal is limited to the front steps and the main front sidewalk; any requests for side areas must be specifically approved by a CIC board member. While we strive to help as many residents as possible, participation is subject to volunteer availability. For any urgent concerns, please contact us at the provided phone number.

Nos complace anunciar que nuestro Programa de Remoción de Nieve, impulsado por voluntarios, ya está en funcionamiento. Community Innovation Center Snow Removal Program Liability Waiver

 

Participant Liability Waiver and Release Form

This Liability Waiver and Release Form (“Waiver”) is executed by the undersigned participant (or the legal guardian of the participant if under 18 years of age), in favor of the Community Innovation Center NFP (“CIC”), its officers, directors, employees, agents, volunteers, and affiliates.

 

1. Acknowledgment of Risk:

The participant acknowledges that participation in the CIC Snow Removal Program is voluntary and involves activities that may include physical exertion and inherent risks, such as slips, falls, and exposure to cold weather conditions.

2. Release of Liability:

In consideration of being allowed to participate in this program, the participant hereby releases, waives, and discharges the Community Innovation Center, its officers, directors, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of any injury, loss, or damage that may be sustained by the participant while participating in the program.

3. Indemnification:

The participant agrees to indemnify and hold harmless the Community Innovation Center and its representatives from any loss, liability, damage, or costs they may incur due to the participant’s participation in the program.

4. Medical Treatment:

The participant consents to receive any medical treatment deemed necessary in the event of injury during participation and agrees that they are responsible for the cost of any such treatment.

5. Understanding and Acceptance:

By signing below, the participant affirms that they have read and fully understand this waiver and release of liability, and agree to its terms .

Illinois Law Notice

This waiver is intended to be as broad and inclusive as permitted by the laws of the State of Illinois. If any portion of this waiver is held invalid, the remaining provisions shall continue in full legal force and effect.
