By signing below, the business and each undersigned owner or principal (“Applicant”) authorizes the party receiving this application, and its successors, assigns and designees, including other brokers and funding intermediaries (“Broker”), to share, transmit, and reproduce this application and all supporting documents for the purpose of assisting the Applicant in obtaining commercial financing and funding products. If a funding intermediary or source requires a different application format, Applicant appoints Broker as its limited agent to complete and submit the information contained in this application and its supporting documents on such form, and Applicant adopts the signature below as Applicant’s authorized, valid, and binding signature which Broker may apply on its behalf on each such submission. This authorization shall continue until revoked by Applicant in writing.
The Business and the Business Owner(s) identified above (individually, an “Applicant”) each represents, acknowledges and agrees that all information anddocuments provided to Enterprise Finest Solutions LLC ("EFS") are true, accurate and complete, Applicant authorizes EFS to disclose all personal and/or businessinformation and documents that EFS may obtain, including credit reports and personal information and any information that Applicant may provide to EFSApplicant’s application(s) for any product, service or program through EFS, to other persons, entities or third-parties that facilitate financing (collectively,“Assignees and Third-Parties”), as follows: (a) to Assignees and Third-Parties that may make, be involved with, or acquire commercial financing programs havingdaily, weekly, bi-weekly or monthly repayment features, (b) to Assignees and Third-Parties that may be involved with purchases of future receivables includingMerchant Cash Advance transactions, and (c) to Assignees and Third-Parties for the purpose of offering Applicant our products, services or programs or those ofthird parties that EFS LLC believes Applicant may find of interest, including, without limitation, equipment finance and/or equipment leasing programs andservices (collectively, “Transaction”) and each Assignee and Third-Party is authorized to use such information and documents, and share such information anddocuments with other Assignees and Third-Parties, in connection with potential Transactions, each Assignee and Third-Party will rely upon the accuracy andcompleteness or such information and documents, EFS, Assignees, Third-Parties, and each of their representatives, successors, assigns and designees(collectively, “Recipients”) are authorized to request and receive any investigative reports, credit reports, statements from creditors or financial institutions,verification of information, or any other information that a Recipient deems necessary, Applicant waives and releases any claims against Recipients and anyinformation-providers arising from any act or omission relating to the requesting, receiving or release of information
