STREAM Play Café – Liability Waiver & Participation Agreement

1. Assumption of Risk
By entering STREAM Play Café, I acknowledge that play activities may involve:

movement
climbing
sensory exploration
toys and materials
interaction with other children
I understand that minor injuries (such as bumps, bruises, or falls) may occur during normal play, and I voluntarily accept all inherent risks.

2. Parent/Guardian Supervision Required
STREAM Play Café is not a drop-off facility.
A parent or responsible guardian must supervise their child at all times and ensure safe and appropriate use of all toys, materials, and equipment.

3. Wellness Agreement
I confirm that my child is healthy today and free of:

fever
vomiting
diarrhea
contagious illness
unexplained rash
If my child becomes unwell while on the premises, we agree to leave the play area promptly for the wellbeing of all guests.

4. Equipment Use & Safety Rules
I agree that:

Children will use equipment as intended
Rough play, pushing, running, or unsafe climbing is not allowed
I will keep younger children away from small materials not age-appropriate
STREAM Play Café is not responsible for injuries resulting from misuse of toys or equipment.

5. Release of Liability
I understand that participation at STREAM Play Café involves ordinary play risks, and I accept those risks on behalf of myself and my child. To the fullest extent allowed by law, I release and hold harmless STREAM Play Café, its owners, staff, affiliates, and partners from claims, demands, damages, losses, or expenses arising out of injury, accident, illness, or property damage occurring during our visit, except where caused by proven gross negligence or willful misconduct.

6. Personal Belongings
I understand that STREAM Play Café is not responsible for any:

lost
stolen
damaged
personal items.

7. Photo/Media Consent
(Selected on the signature form)

YES — I allow STREAM Play Café to use photos/videos of my child for marketing.
NO — Do not use photos/videos of my child.

8. Food & Beverage Policy 
No outside food is allowed.

Outside beverages may be brought in only if they are in a personal beverage container
(such as a tumbler, reusable bottle, or child’s lidded cup).

Store-bought drinks (like Coke, Sprite, Starbucks, etc.)
must be poured into a personal container before entering the play area.

Approved snacks and beverages are available for purchase at the café.

9. Special Events & Parties
Party deposits are non-refundable but may be transferred with advance notice
All party guests must follow STREAM Play Café safety rules
A full party policy will be provided during booking.

10. Agreement
By signing the digital form, I acknowledge that I have read, understood, and agree to this waiver.

Children must wear socks and appropriate footwear in the play area to support safety and cleanliness.

STREAM Play Café reserves the right to ask any guest to leave without refund if behavior is unsafe, disruptive, aggressive, destructive, harassing, or otherwise poses a risk to other guests, staff, or property. Failure to follow posted rules, staff instructions, or safety policies may result in immediate removal from the premises.

Food and beverage products sold, served, or consumed at STREAM Play Café may contain or come into contact with common allergens including but not limited to milk, eggs, peanuts, tree nuts, soy, wheat, and other allergens. STREAM Play Café cannot guarantee an allergen-free environment.

Admission to STREAM Play Café is subject to occupancy and safety capacity limits. STREAM Play Café reserves the right to refuse entry, delay admission, or limit participation when capacity has been reached or when necessary for guest safety and comfort.

By signing this waiver, I authorize STREAM Play Café staff to seek emergency medical treatment for my child if I cannot be reached.

In the event of a medical emergency, the parent or guardian authorizes STREAM Play Café staff to contact emergency medical services and obtain emergency medical treatment for their child if necessary, and acknowledges responsibility for any resulting medical expenses.

For safety, cleanliness, and guest comfort, shoes must be removed in designated play areas. Approved socks may be required for participation in certain play spaces and activities. Guests who do not comply with footwear requirements may be restricted from entering designated play areas.
