I authorize Kemlo Transport LLC to obtain my Motor Vehicle Record (MVR) for the purpose of evaluating my driving history in connection with employment, insurance, or other lawful purposes. I understand that this record may include information about my driving history such as violations, accidents, suspensions, and license status for the past three (3) years.

I release Kemlo Transport LLC, its agents, and any individuals or organizations that provide information at my request from any liability in connection with this authorization.
