I acknowledge that participation in dance classes, performances, rehearsals, or any related activities at Opal Performing Arts involves physical activity that may result in injury. I understand that, despite the precautions and safety measures taken by Opal Performing Arts, students participate at their own risk and no liability will be taken from Opal Performing Arts for any injury that occurs on our studio premises.

I, hereby release and discharge Opal Performing Arts, its owners, staff, agents, volunteers, and affiliates from any and all liability for any injury, illness, accident, or damage to property or person that may occur from participating in dance classes, performances, rehearsals, or any related activities at Opal Performing Arts.

I give permission for the staff at Opal Performing Arts to administer first aid on my child, within the limits of their certified first aid training. This may include, but is not limited to, the application of band-aids, ice packs, and support for minor injuries such as sprains, strains, or suspected fractures.

In the event of a serious or medical emergency, I understand that staff will call for an ambulance and that I am responsible for any associated costs. Every effort will be made to contact me or the listed emergency contact as soon as possible.

I understand that I may revoke this release at any time by providing written notice to Opal Performing Arts. Revocation will apply to future use but will not affect any prior use that already occurred.
