In the event that a member in good standing (dues current) is unable to attend a regular or special meeting of the NCDHA General Assembly and wishes to have a vote cast on his/her behalf on matters coming before the General Assembly of the NCDHA, he/she may assign proxy to their Component Trustee. Proxy guidelines are as follows:
· Official Proxy form must be used with proper date indicated
- Email requests, text requests, voicemail requests will not be honored;
· Official Proxy form must be signed and dated by the member and witnessed by another Registered Dental Hygienist or by a Notary Public;
· Official Proxy form must be submitted to the NCDHA office by 5PM not less than 7 full days prior to the General Assembly meeting (see deadline below);
· Official Proxy form MUST be submitted ONLY by the member requesting the proxy vote;
· Forms may be submitted via email, via facsimile, or by mail.
· Proxy is assigned ONLY for the meeting date indicated on this form;
· While you may share your thoughts/views with your Component Trustee on matters that are known to be addressed in advance, please understand that the trustee may have matters presented at the meeting that you will not have access to and he/she will be entitled to vote on your behalf as they see fit after hearing the discussions and amendments presented at the meeting...acknowledging that their views also may not be the same as yours;
· Once you assign your proxy, it cannot be changed or revoked;
· Failure to follow ALL guidelines and/or failure to provide all required information will result in your proxy vote being declined;
· It is your responsibility to notify your Component Trustee that you are submitting and assigning your proxy to him/her and to get meeting updates on action items from your Component Trustee;
