    For new clients, this information is essential to ensure accurate tax filing. Please submit at least your prior-year tax return (preferably two years) for comparison.

    For existing clients, we kindly ask that you provide this information again to keep our records accurate and up to date. You may indicate sections that remain unchanged from last year to avoid re-entering details. IRS compliance is crucial, and returning clients know we work hard to secure every dollar you deserve.

    IMPORTANT: A checklist of commonly required documents is included. You may save your progress and return anytime while gathering your documents.     This letter outlines the terms of our engagement and the services we will provide. Your signature confirms that you understand and agree to the terms below.

    By signing, you acknowledge that the information you provide will be submitted to the IRS and state tax authorities, and that providing false or misleading information is illegal and subject to penalties. You understand that JACKSON & CO FINANCIAL is responsible only for errors or omissions on our part, including related interest and penalties—not any tax due.

  • OUR RESPONSIBILITIES

    • We prepare your federal, state, and local tax returns based on the information you provide.
    • We do not audit or verify your data, but we may request clarification.
    • We will work to identify all credits and deductions you are legally entitled to.
    • Additional bookkeeping fees may apply if needed.
    • We are not responsible for detecting fraud or irregularities.
  • YOUR RESPONSIBILITIES

    • Provide all necessary information for accurate and complete tax returns.
    • Respond promptly to any requests for clarification.
    • Keep all documents that support your income, expenses, deductions, and credits.
    • Review your return carefully before signing.
    • Ensure all information you provide is truthful.

    You are ultimately responsible for the accuracy of your tax return.

  • PENALTIES, EXAMINATIONS, AND NOTICES

    The IRS and state tax authorities may impose penalties for issues such as underreported income, late filing, underpaid estimated taxes, or insufficient tax withholding. Any return may also be selected for examination.

    Taxpayers may face additional penalties if they understate their tax liability.

    Your federal or state return—or both—may be chosen for review. Any adjustments proposed by an examining agent may be appealed through the appropriate channels. Optional audit protection is available for an additional fee.

  • PRIVACY POLICY
    We protect your personal information according to professional confidentiality standards. We share information only as required by law or as needed to provide services, and only with parties who agree to safeguard your data.

    We maintain physical, electronic, and procedural security measures for all client records, including former clients.

  • OUR FEES
    Fees are based on the time, forms, schedules, and work required—not on your refund amount. Payment is required whether or not you receive a refund.

  • ARBITRATION
    If a dispute arises and cannot be resolved through discussion, both parties agree to attempt mediation through the American Arbitration Association before considering arbitration or litigation.

  • AGREEMENT
    By signing, you confirm that all information you provide is true, correct, and complete, and you understand these terms will continue to apply in future years unless updated.

  • We ask that you complete this due diligence questionnaire to ensure you are eligible to claim at least one of several valuable tax credits.  While the questions may seem to be a little intrusive, they are questions that we are required to ask under the due diligence requirements of the new tax laws.  We will keep your responses and documents on file, in the event the IRS should ever ask for them.   • Approval rate of 79%. Your refund and loan history is reviewed from previous years. 4 out of 5 of clients are approved. 

    **Loans over $1000, have an 36% interest fee attached.

    The maximum amount for a Refund Advance Loan is $7000. If approved for a Refund Advance Loan greater than $1,000, you will be charged interest. The amount of interest you are charged depends on how many days it takes to repay the advance from your tax refund. In other words — if your tax refund is processed and deposited by the IRS or State quickly, you’ll pay less than if your tax refund takes longer to be released. Interest charges are capped at 60 days. So, the charges to you will stop at 60 days if your refund is delayed.

Unless authorized by law, we cannot use your tax return information for purposes other than the preparation and filing of your tax return without your consent. You are not required to complete this form to engage our tax return preparation services. If we obtain your signature on this form by conditioning our tax return preparation services on your consent, your consent will not be valid. Your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature. For your convenience, we have entered into an arrangement with certain companies to offer an Electronic Refund Disbursement Service and/or Loan product. To determine whether these offerings may be of interest to you, we will need to use your tax return information. If you would like us to use your tax return information to determine whether these offerings are relevant to you while we are preparing your return, please sign and date this consent to the use of your tax return information. By signing below, you authorize us to use the information you provide to us during the preparation of your tax return to determine whether to offer you an opportunity to apply for the Electronic Refund Disbursement Service and/or Loan product.


    Consent to Disclosure of Tax Return Information

Unless authorized by law, we cannot disclose your tax return information to third parties for purposes other than the preparation and filing of your tax return without your consent. If you consent to the disclosure of your tax return information, Federal law may not protect your tax return information from further use or distribution. You are not required to complete this form to engage our tax return preparation services. If we obtain your signature on this form by conditioning our tax return preparation services on your consent, your consent will not be valid. If you agree to the disclosure of your tax return information, your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature. You have indicated that you are interested in utilizing the Electronic Refund Disbursement Service and/or Loan product from [Refund Advantage Bank]. To provide you with the opportunity to apply for and/ or receive the Electronic Refund Disbursement Service and/or Loan product, we must disclose all of your tax return information to [Refund Advantage Bank]. You may request a more limited disclosure of tax return information, but you will not be eligible to submit an application for the Electronic Refund Disbursement Service and/or Loan product.     By signing below, you authorize us to disclose to [Refund Advantage Bank] all of your tax return information so that it can evaluate and process your application for the Electronic Refund Disbursement Service and/or Loan product. You understand that if you are not willing to authorize us to share your tax information with [Refund Advantage Bank], you will not be able to obtain the Electronic Refund Disbursement Service and/or Loan product, but you can still choose to have your tax return prepared and filed by us for a fee. If you believe your tax return information has been disclosed or used improperly in a manner unauthorized by law or without your permission, you may contact the Treasury Inspector General for Tax Administration (TIGTA) by telephone at 1-800-366-4484, or by email at complaints@tigta.treas.gov. 

     

    Consent for Payment


    [JACKSON & CO FINANCIAL] is not responsible in the event the taxpayer omits or provides incorrect information (i.e. social security numbers, names, dates of birth, etc.).  These errors may delay your refund. [MIKE & CO FINANCIAL] does not control whether or not the IRS randomly selects your return for an audit, however all clients are welcome to purchase Audit Protection and we will always keep all necessary documents on file to dispute an audit on your behalf.  All information obtained from the taxpayer and/or spouse must be submitted if the IRS audits your tax return. 

    [JACKSON & CO FINANCIAL] is not responsible for any incorrect tax figures provided by the taxpayer and/or spouse.  If your tax figures change you will need to submit an amendment.  [JACKSON & CO FINANCIAL] offers amendment preparation for an additional fee.  If you have any overdue debts (i.e. school loans, child support, DPP, DFCS, etc….) that are scheduled to offset your refund, you will be required to pay your tax preparation fees upfront.  You are able to call the IRS offset department at 1-800-304-3107 or 1-800-829-7650 to check the status of any scheduled offsets. If you are unaware of a potential offset and your refund is completely garnished, your prep fee will be drafted from the bank account you have on file with our office. 

    [JACKSON & CO FINANCIAL] is not responsible for any changes the IRS or receiving bank may make on disbursement dates.  

    [JACKSON & CO FINANCIAL] is not responsible for any glitch issues that the IRS experiences that may cause a delay in receiving your tax refund. We DO NOT reimburse any bank fees in the event of this occurrence.

     

    MUTUAL NON-DISPARAGEMENT


    [JACKSON & CO FINANCIAL] agrees to refrain from any disparagement, defamation, libel, or slander of any of the Releasees, and agrees to refrain from any tortious interference with the contracts and relationships of any of the Releasees. Client agrees to refrain from any disparaging statements about [JACKSON & CO FINANCIAL]. Any instances of this found by [JACKSON & CO FINANCIAL] will result in a $1000 penalty per occurence payable by the client to [JACKSON & CO FINANCIAL].

     

    Clerical/Banking Errors


    [JACKSON & CO FINANCIAL] tries its best to ensure that all client information is entered correctly. In the event a clerical error is dicovered that causes an incorrect deposit into your account, you give [JACKSON & CO FINANCIAL] permission to debit your account for the amount of the deposit to reverse this error.
