I acknowledge that I have received my dress, including any completed alterations and accessories, from Silhouette Bridal & Formal. I confirm that I have fully inspected all items at the time of pick-up and that everything is accurate, complete, and in satisfactory condition.
I understand that all sales are final. Dresses are eligible for store credit only within 3 days of purchase, except wedding gowns and any altered or custom-fitted items, which are not eligible for return, exchange, or store credit under any circumstance.
I also understand that once the dress and related items leave Silhouette’s premises, the store is not responsible for any damages, issues, or concerns that may arise thereafter. By signing below, I accept full responsibility for the dress and all accompanying items upon leaving the store.
