Waiver & Consent Agreement - Silhouette Dance Company
Participation in any Silhouette Dance Company activity and use of any recreational facilities involves a risk of accidental injury despite all safety precautions. Having been informed of the activities conducted by the Silhouette Dance Company, I/we, as an individual or as a parent or guardian of the participants named herein, assume all risks and hazards incidental to the activities, and release from responsibility and agreed to indemnify and hold harmless Silhouette Dance Company, its officers, directors, independent contractors, volunteers and all employees for any illness or injury to me or my children or family members occurring during his/her/our participation in any activity or use of any recreational facility at or conducted by Silhouette Dance Company.
I understand that Term fees are due by end of week 4 of each term to retain the applicable discount. Payments received after the end of week 4 of each term result in forfeit of discount. Casual payments are due prior to each class.
By signing herewith I agree to be bound by Silhouette Dance Company Terms of Trade and Policies.
In the event the Studio refers an outstanding debt to a debt collection agency and/or law firm for collection of the outstanding balance, the Student/Parent (person nominated for being responsible for paying account) shall be liable for the recovery costs incurred, and if the agency charges commission on a contingency basis the Student/Parent (person nominated for being responsible for paying account) shall be liable to pay as a liquidated debt, the commission payable by the Studio to the agency, fixed at the rate charged by the agency from time to time as if the agency has achieved one hundred per cent recovery.
