Liability Waiver and Release Form

1. Acknowledgment of Risk

I, the undersigned, acknowledge and understand that there are inherent risks associated with participating in music classes and related activities. I understand that these risks include, but are not limited to, physical activity, potential for accidents, and other unforeseen incidents.

2. Waiver and Release

In consideration of my child’s participation in the music class, I hereby agree to release, waive, discharge, and hold harmless Miss Brooke Music LLC , its employees, agents, and representatives from any and all claims, liabilities, damages, or expenses arising out of or related to any injury, accident, or incident that may occur while my child is participating in the music class or on the premises.

3. Assumption of Responsibility

I understand and agree that I am responsible for my child’s safety while attending the music class. I also understand that Miss Brooke Music LLC will not be held responsible for any injuries or damages that occur as a result of participation in the class or while on the premises.

4. Emergency Medical Treatment

In the event of an emergency where medical treatment is required, I consent to Miss Brooke Music LLC seeking medical treatment on behalf of my child. I understand that I will be responsible for any medical expenses incurred.

5. Photography and Media Release

I give permission for Miss Brooke Music to take photographs and/or video recordings of my child during the music class for promotional purposes, including but not limited to social media, websites, and other marketing materials.

6. Governing Law

This waiver and release form shall be governed by and construed in accordance with the laws of the state of Pennsylvania, without regard to its conflict of laws principles.

By signing below, I acknowledge that I have read and understood the terms of this Liability Waiver and Release Form. I agree to the terms and conditions set forth and give my consent for my child to participate in the music class.

Photo Release Form- Not signing shows you do not consent to any pictures/videos of your child.

Hereby agree and consent as follows.

1.    I consent and authorize Miss Brooke Music LLC, located at 340 Media Station Rd, Media, PA 19063 to use my likeness in any photograph, video, or other digital media (“Photos”) taken or to be taken during class of them and their child while participating in Brooke Ross’s Children’s class for the purpose of advertisement, in any and all of its publications, including print or web-based publications.

2.    I irrevocably authorize Miss Brooke Music LLC to copy, edit, enhance, crop, or otherwise alter any photo for use in their publications. I also waive any rights for approval or inspection of any photos.

3.    I understand and agree that all photos are the property of Miss Brooke Music and may not be returned to me.

4.    I acknowledge that I am not entitled to any compensations or royalties with respect to the use of the photos.

5.     I agree to release and forever discharge Mis Brooke Music LLC and its affiliates, successors and assigns, officers, employees, representatives, partners, agents anyone claiming through them, in their individual and/or corporate capacities from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action of any nature or kind, known or unknown, which I, and anyone claiming on behalf of me, may have or claim to have against Release in connection with this Release.

6.     I have carefully read and fully understand all the provisions of this Photo Release Form and am freely, knowingly, and voluntarily signing.
