2026 Orange Field of Valor Flag Tribute Order Form
THREE-YEAR FLAG TRIBUTES AVAILABLE FOR 2026, 2027 AND 2028! When you get to the payment submission form, you will have the option to purchase a 3 year renewal of your Flag Tribute for a discounted rate of $30/flag per year ($90 for a three-year renewal). Your Flag Tribute will be automatically renewed in 2027 and 2028--you will not need to resubmit a form unless information has changed.
Please note, if you do not have all the information we are asking for, it is okay! You can still submit a form, providing as much information as you can. Please note that once you begin filling out this form online you cannot save it for later, so please gather your information before you start. If you need to make corrections after you submit, please email susie@communityfoundationoforange.org.

When listing awards received, if you can list them in order or precedence, it would help us so that we don't have to look it up and risk getting it wrong! Also, if you can avaoid using abbreviations or acornyms, that would be helpful. (ie--instead of NDSM please write National Defense Service Medal) Thank you for helping!
