Media Release & Photo Consent Policy
DFW Rise Youth Sports

To support the mission of DFW Rise Youth Sports and celebrate the growth and success of our youth participants, we may occasionally capture photographs and videos during games, practices, community events, and team functions. These images may be used to promote the organization through printed materials, social media, websites, press releases, and other communication platforms.

Consent & Release:
By signing this form, I authorize DFW Rise Youth Sports, its officers, agents, coaches, staff, volunteers, and authorized media representatives to:

Photograph, record, or videotape my child during activities organized or sponsored by the organization.
Use my child’s image, voice, name, or likeness in promotional and educational materials related to DFW Rise Youth Sports, including:

Websites, social media accounts, brochures, flyers, and newsletters
Local media coverage (TV, newspapers, radio)
Fundraising and sponsorship materials

Conditions:
I understand that all photos and recordings will be used in a manner consistent with the values of DFW Rise Youth Sports.
I acknowledge that my child’s participation is voluntary and that no compensation will be provided for the use of any media.
I understand that I may revoke this consent in writing at any time by contacting the organization, though revocation does not apply to past publications.

PARTICIPANT WAIVER & MEDICAL RELEASE FORM

This form must be acknowledge and signed by a parent or legal guardian prior to participation in any DFW Rise Youth Sports events.

I, the undersigned, as the parent/legal guardian of the participant listed below, hereby give my full consent for my child to participate in any and all activities, training sessions, games, camps, or events organized by DFW Rise Youth Sports.

I understand that participation in youth sports may carry a risk of injury. I acknowledge and fully understand that my child may be engaging in activities that involve risk of injury, including but not limited to sprains, fractures, heat exhaustion, and other physical ailments. I, for myself and my child, hereby waive, release, and hold harmless DFW Rise Youth Sports, its coaches, volunteers, directors, sponsors, facility partners, and all associated personnel from any and all liability, claims, demands, or causes of action for injuries, losses, or damages that may arise during participation in the program.

I further authorize DFW Rise Youth Sports staff or volunteers to seek emergency medical treatment for my child in the event of injury or illness if I or the listed emergency contact cannot be reached. I understand that every effort will be made to contact me before such action is taken. I assume full responsibility for any medical expenses incurred as a result of emergency care.

REFUND POLICY FOR REGISTRATION, UNIFORMS, & FEES

Thank you for choosing DFW Rise Youth Sports. To ensure the integrity of our program and proper planning for uniforms, equipment, and league participation, we ask that all families review and agree to the following refund policy:

ALL SALES ARE FINAL – NO REFUNDS

Once your child is registered and uniform or participation fees are paid:

There are absolutely ❌ no refunds for any reason, including but not limited to injury, withdrawal, relocation, or schedule conflicts.

This includes but is not limited to:
▪ Uniform orders (jersey, pants, socks, etc.)
▪ Registration fees
▪ League or tournament entry contributions
▪ Team apparel or add-ons

Why no refund

Our program immediately allocates funds to cover the cost of:

Customized uniform orders (non-refundable from the supplier)
Insurance, league registration, and facility rental fees
Equipment and team materials ordered based on registration counts
Operational costs committed based on total headcount
Once these commitments are made, funds are not recoverable by the organization, which is why we must maintain a strict policy.

Chargeback & Dispute Policy

By completing payment, you agree to these terms in full.
Any attempted chargebacks or disputed payments will be met with documentation including:

This signed and acknowledged policy
Timestamped proof of registration
Confirmation of uniform order and associated costs
Disputes filed in violation of this policy may result in legal follow-up and removal from future participation in the program.

Parent Code of Conduct – Fall 2026 Season

Our Commitment
At DFW Rise Youth Sports, we believe in nurturing not just athletic skills but also character, discipline, and respect. As parents and guardians, your behavior sets the tone for our young athletes. We ask for your partnership in modeling the values we aim to instill in our players.

Expectations for Parents and Guardians

Lead by Example Policy

Demonstrate sportsmanship and fair play at every game and practice.
Encourage your child and their teammates with positive reinforcement.

Respect All Participants

Treat coaches, officials, players, and other spectators with respect, regardless of the game's outcome.
Refrain from using profanity, derogatory language, or gestures.

Support the Coaching Staff

Trust the coaching decisions and strategies implemented for the team's benefit.
Avoid coaching from the sidelines; allow coaches to guide the players.

Maintain a Positive Environment

Focus on the enjoyment and development of the game rather than the score.
Celebrate effort, improvement, and teamwork.
Address Concerns Appropriately

If issues arise, discuss them privately with the coach or appropriate staff member at a suitable time.
Avoid confrontations during games or practices.
Prohibited Behaviors

To ensure a safe and supportive environment, the following behaviors are strictly prohibited:

Verbal or physical abuse towards anyone present at games or practices.
Entering the field of play during games or practices without permission.
Consumption of alcohol or illegal substances on or near the premises.
Engaging in arguments or confrontations with officials, coaches, or other spectators.
Consequences of Misconduct

Violations of this code may result in:
Permanent removal from the DFW Rise Youth Sports program.
Our priority is the well-being and development of our young athletes. We appreciate your cooperation in upholding these standards.

TRAVEL

Some games during the Fall 2025 season may require travel to surrounding cities within the Dallas-Fort Worth metroplex. While we strive to keep the majority of events local, participation in league play may involve travel to nearby fields for away games or tournaments. By registering my child, I acknowledge and accept that travel may be required as part of the team’s schedule.

By signing and submitting this registration, I acknowledge that I have read, understood, and agree to all terms outlined in this form, including:

The Parent Code of Conduct
The Medical Authorization & Emergency Treatment Release
The Liability Waiver for participation in sports-related activities
