- Please read each statement carefully regarding our Foster Care Agreemnt and Foster Program. Checking each box indicates that you understand and agree to abide by each statement.
- I, the Caretaker, also referred to as the Foster Parent or Foster Home, am the primary caretaker for the foster animal(s) and am 21 yrs of age or older.
- I do not have any right or authority to give, transfer, or adopt out custody of foster animal(s) to any other person, organization, unless approved by Made a Promise Rescue.
- I agree that the foster animal will not be left in the care of or allowed to go with any individual outside of my household without prior approval from Made a Promise Rescue
- I understand that Made a Promise Rescue is the sole and rightful owner of the animal(s) in foster. All decisions regarding the animal(s) shall be made by Made a Promise Rescue.
- I agree to return said animal(s) with 24 hours upon request of Made a Promise Rescue or if I am no longer to care adequately for them. If for any reason I am unable to continue to house, keep separate and care for the foster animal(s); I will notify Made a Promise Rescue immediately and will return the animal(s) to Made a Promise Rescue.
- Made a Promise Rescue reserves the right to inspect the foster home and property at an time.
- I understand that Made a Promise Rescue may not have a place for my foster dog to go if being returned for any reason. Made a Promise Rescue may ask that the foster home continues to hold onto the animal(s) until other placement can be arranged. The animal(s) may be placed into a new foster home, boarding facility, or euthanize if no other solution is found.
- I attest to have both experience and competence to handle and care for the foster animal(s)
- I understand for the duration of the temporary custody, the animal(s) become the responsibility of the Foster Parent and is subject to all the Rules, Ordinance and Regulations of the City Municipality, Town, Township, Borough, Village, County, and the laws of the State wherein the Foster Parent and the animal(s) resides.
- If renting/owning, I acknowledge that leasing and/or HOA agreements allow the animal(s) at the property. I understand I may be asked for this documentation by Made a Promise Rescue.
- I understand that all veterinary care must be approved by Made a Promise Rescue in advance.
- I agree that the animal(s) shall be provided the following: A) Reasonable exercise in a securely fenced yard appropriate for the animal(s), dog run, or on a leash. B) Chaining and tethering shall not be used as a primary means of confinement. C) The animal(s) shall be always under immediate control and protection of the Foster Parent and will not be permitted to roam outside of the fenced area or quarantine room.
- I agree that the animal(s) shall be supervised at all times by the foster parent, weather indoors or outdoors. Foster animals must be supervised at all times when children or other animals are present.
- I agree that I have been given policies and procedures necessary to properly introduce foster animals to other animals and children in the home. If I have not been made aware of policies and procedures, I under that it is my responsibility to request and review and policies and procedures.
- I agree to keep the foster animal indoors as a household pet and may not be kept outdoor. I further agree to provide proper care at all time, including adequate food, fresh water, and a safe environment that supports the animal's physical, mental, and emotional wellbeing. I agree to promptly notify Made a Promise Rescue of any medical concerns, changes in health, or veterinary needs. All medical care must be approved and coordinated by Made a Promise Rescue unless otherwise authorized in writing.
- I agree to incorporate the animal(s) into my daily routine; and to create and leash train, housetrain, give medication(s), socialize, and work with any behavioral problems including but not restricted to appropriate obedience. All uses of training equipment must be approved by Made a Promise Rescue.
- I agree to keep Made a Promise Rescue updated on the health and behavior of the foster animal(s) in my care. In addition, I agree to respond to any inquires about my foster animal(s) in a timely manner.
- If an animal is harmed due to my negligence, I understand that I will be held responsible for the possible bills of vetting, surgery, and/or meds.
- I understand that Made a Promise Rescue is not responsible nor will be held liable for any damages to property or possessions caused by the foster animal(s) .
- I agree not to have the animal(s) interact with other dogs, animals, or children in the home until the animal(s) and the other dogs, animals, or children have become acquainted and approval has been granted by Made a Promise Rescue. I agree to be present and have visual contact with the animal(s) and when loose around other dogs, animals, and children.
- I understand that Made a Promise Rescue suggests that I keep animal(s) in a closed off area segregated from my current pets for a designated period of time in my care. There are risk that my pet(s) can get sick from the foster animal(s). I understand that Made a Promise Rescue cannot be held responsible for any illness that is passed along to owned pets. Made a Promise Rescue will not be held financially responsible for any vetting or medication that my pet may need.
- I agree to feed my foster animal(s) separately from any other pets living in the residence.
- I agree to supervise toys, food, and/or any other high value item(s) between my foster animal(s) and from any other pets/people/children living in the residence.
- I agree to keep animal(s) in clean surroundings and to perform routine grooming and brushing to further maintain the animal(s) in good condition.
- I understand that there will be learning opportunities available by Made a Promise Rescue to help with my foster needs. I will keep in contact with Made a Promise Rescue with regular updates and if any questions or concerns arise.
- I acknowledge that Made a Promise Rescue or any volunteer(s) of Made a Promise Rescue have not made any warranties regarding the future condition, temperament, or conduct of this animal(s). I agree and understand that an animal can be unpredictable and that Made a Promise Rescue cannot anticipate or insure against unexpected conduct of an animal fostered from Made a Promise Rescue.
- I acknowledge that Made a Promise Rescue is not liable for any acts of a foster animal(s) while in the custody of the foster parent.
- I agree to immediately contact Made a Promise Rescue in the event of an emergency. An emergency is defined as escape, stolen animal, bite to a human or another animal, or medical emergency (example: ingestion of a toxin).
- I agree to notify Made a Promise Rescue within 24 hours of ant change in address, phone number or e-mail address.
- I agree to return any supplies provided to me to Made a Promise Rescue. These are borrowed items (example: dog kennels) and must be returned to Made a Promise Rescue when foster parent ceases fostering for Made a Promise Rescue. Foster parent may purchase said item(s) at a price agreeable to Made a Promise Rescue and Foster Parent, upon Made a Promise Rescue's approval.
- I agree to hold the Made a Promise Rescue harmless from any direct or consequential damages arising out of this foster care arrangement. I hereby accept this animal(s) and assume all the risks and responsibilities associated with this animal(s); including bites, and fully and completely release, indemnify and hold harmless, Made a Promise Rescue, its owners, directors, volunteers and agents from any claim, cause or action or liability or an sort of nature, whether, known or unknown, directly, or indirectly arising out of or in connection with animal(s) fostered.
- I acknowledge that the effective date of this Agreement is the date upon which signer/volunteer/caretaker first provided/will first provide services for Made a Promise Rescue, and shall remain in effect until Signer/Volunteer/Caretaker ceases to perform services for Made a Promise Rescue.
