To participate in the Baltimore City Fire Department Junior Cadet Program.

I {parentguardianName} understand that the program will include educational activities, station visits, demonstrations and mentorship opportunities with fire department personnel. Participation requires adherenceto program rules, code of conduct, and attendance expectations.

I give permission for representatives of the Baltimore City Fire Department to provide transportation to my child for program-related events.

In the event of an emergency, I authorize first aid and emergency medical care if necessary.

I understand that the program may include minimal-risk, hands-on training with trained supervision.

I understand that photographs or videos may be taken and authorize their use in BCFD materials.
