The Arts Center’s 4K class is a partnership with the Sheboygan Area School District. Official Sheboygan Area School District registration will take place in August, 2026. Completion of this form does not guarantee a spot in the 4K class.

You will be notified as to the placement of your child as soon as possible. In addition to this form you will need to complete a school choice form (or Alternative Open Enrollment form if you live outside of the SASD) required by SASD available on the SASD website.

Reminder: you may not enroll your child in two SASD schools for the same year.
