Waiver and Release of Liability In consideration of being permitted to enter and use the indoor playground and equipment at Play Gallatin, the undersigned acknowledges and agrees to the following terms: 1. Supervision and Responsibility I understand and agree that I am solely responsible for supervising the child(ren) listed above during our visit. Children must be under the direct supervision of their parent, legal guardian, responsible adult or caregiver at all times while on the premises. Play Gallatin and its staff are not responsible for monitoring or supervising children.

I acknowledge that alcoholic beverages may be available and served within the facility. I understand that it is my responsibility to ensure my own conduct remains safe and appropriate and that the child(ren) in my care remain supervised and protected at all times. I agree that consumption of alcohol will not impair my ability to monitor, care for, and ensure the safety of the child(ren) I am responsible for. 3. Assumption of Risk I understand that participation in activities (the "Activities") at Play Gallatin involves inherent risks, including but not limited to falls, collisions, equipment use, acts by third parties or contact with other participants which may result in injuries. I voluntarily assume all such risks, both known and unknown (the "Risks"), associated with participation. 4. Release of Liability In exchange for the right to participate in the Activities, I hereby assume all responsibility and liability for these Risks, whether known or unknown, direct or indirect. On behalf of myself, my family, minor participants for whom I am responsible and any successors and assigns, I hereby release, waive, discharge, and hold harmless, Play Gallatin, its owners, agents, employees, and/or subcontractors, (collectively referred to as "Releasees") from and against any and all claims, demands, liabilities, controversies or causes of action, damages, costs, and/or expenses of any kind or nature whatsoever, that may hereafter accrue, relating to or arising out of the Activities, my participation in the Activities, and/or the Risks.

5. Indemnification and Hold Harmless

I, on behalf of myself, the child(ren) listed above, and our respective heirs, assigns, and representatives, hereby indemnify and hold harmless Play Gallatin, its owners, staff, agents, and affiliates from any and all claims, demands, or causes of action

arising out of or related to injury, illness, or damages that may occur while on the premises or participating in any activities.

I agree to follow all posted rules, verbal instructions, and safety guidelines provided by the facility. I will ensure that the child(ren) in my care do the same. I understand that failure to adhere to rules may result in removal from the facility without refund.

Consent and Acknowledgment By signing below, I acknowledge that I have read, understood, and voluntarily agree to this waiver and release of liability, including the acknowledgement of alcohol on-site and my responsibility to ensure safety.
