Safety & Liability Waiver / Medical Consent
— I understand that baseball, strength training, and athletic activity involve inherent risk of injury, including but not limited to: impact from baseballs, bats, equipment failure, slipping or falling, collisions with other participants, overuse injuries, and risks associated with high-effort movement or conditioning.
— I agree not to hold COBALT Athlete, COBALT Baseball, George Washington High School, its coaches, staff, volunteers, or facility owners liable for any injury, accident, or medical condition that may occur during preseason training or while on the premises.
— I understand that it is the player's responsibility to wear a helmet at all times when in the batting cages, soft toss areas, tee stations, or any live/coach-fed hitting environment. Failure to wear a helmet is a violation of safety protocol and may result in removal from the session.
— I understand that the preseason program takes place inside the COBALT Baseball facility. I acknowledge the risks associated with shared cage environments, pitching machines, nets, turf, strength equipment, and balls in motion.
— I understand that participation in this preseason program does not guarantee a roster spot on any GW High School baseball team. Tryouts and team selection are based solely on performance during the official spring season.
— I confirm that my child is physically able to participate in baseball and strength-related activity, or that I have consulted with a medical professional who has cleared them for participation.
— In the event of an injury or emergency, I authorize the coaching staff to seek appropriate medical care for my child and to contact me using the information provided in this form.
— I understand that all payments are final. There are no refunds for any reason, including but not limited to schedule conflicts, missed sessions, illness, change of plans, lack of attendance, or withdrawal from the program.
— I acknowledge that failure to follow instructions, unsafe behavior, or disrespect toward coaches, staff, teammates, or the facility may result in my child being removed from the session or program without refund.
— By signing below, I acknowledge that I have read, understood, and agree to all preseason policies, expectations, safety rules, and liability terms stated in this registration form.
