Funding, if awarded, will be delivered through an electronic payment method chosen by the program administrator. No cash payments will be issued.

Award recipients are solely responsible for any personal tax obligations related to funds received. The program administrator does not provide tax advice.

One recipient will be selected and notified on or around January 31, 2026. The selected applicant will be contacted directly using the email and phone number provided on this form. If the selected recipient does not respond within 48 hours, an alternate recipient may be chosen.
