I certify that my child is physically fit to partake in the activities during the Bigfork Middle School Volleyball Camp. I, the below signed, release Bigfork School District from all claims on account of injury my child may sustain while on school premise. The below signed will not hold Glacier Sky Athletics, its coaches or volunteers responsible in the event of injury, accident or illness.
