This request form should only be filled out 1 time for each church for youth camp and 1 time for each church for kids camp. This form should be filled out by the Group Coordinator, who will be responsible for all communication with the NextGen office and all communication with students and parents.
This is a request form, there is no guarentee that you will receive your preferred week. All requests at the end of the 2 weeks will be given equal consideration. We will be contacting you after February 4th to let you know which week you have been placed in and if you want to change the number of students attending.
If any changes need to be made to your reservation during this 2 week period, please call our office. DO NOT fill out a second reservation request.
