I certify that I am an authorized user of this credit card, and that I will not dispute the payment with my credit card company so long as the transaction corresponds to the terms indicated in this form. I understand there is a 48 business hour cancellation policy (weekends do not count as business hours) requiring me to inform the clinic that I cannot keep my appointment. In case of no shows or "last minute" cancellations within 48 hours, the deposit or any other payments are forfeited.

This membership is non transferable. Should you need a refund without fully using the sessions, each session you have used reverts to the regular booster infusion price of $550. Any late cancellations or no shows result in loss of a membership session.
