BE IT KNOWN that I, {parentguardianName}, the undersigned parent or guardian of {studentName}, hereby give full consent and authorisation to any qualified medical doctor or hospital to render such aid, treatment, or emergency care to my child as may be required on an EMERGENCY BASIS, when such treatment is deemed necessary and when efforts to contact me have been unsuccessful.

IT IS FURTHER UNDERSTOOD that any expense incurred for emergency transportation and/or medical treatment will be the sole responsibility of the parent or guardian. I acknowledge that payment of any such expenses is not the responsibility of Project Learn.

Parental Authorization Required for Participation

I, {parentguardianName}, the parent or legal guardian of {studentName}, hereby grant permission for my child to participate in all activities organised by Project Learn, including but not limited to: on-site programs, electives, field trips, and park days.

I certify that my child is physically, mentally, and emotionally capable of participating in the program. I understand participation is voluntary and that Project Learn staff will take reasonable safety precautions.

In consideration of my child being allowed to participate, I voluntarily and knowingly release Project Learn, including its officers, agents, employees, and affiliates, from any and all liability arising from my child’s participation, including but not limited to:

• All claims (past, present, or future), whether known or unknown
• Personal injury
• Illness
• Death
• Property damage

I understand that outdoor education, travel, and recreational or instructional activities involve inherent risks. I assume full responsibility for any injury, harm, or damage incurred by myself or my child as a result of these activities.

I agree to indemnify, defend, and hold harmless Project Learn, its employees, officers, instructors, and affiliates from all claims, liabilities, damages, or costs (including attorney’s fees) arising out of my child’s participation. This agreement is binding upon me, my spouse, heirs, representatives, assigns, and any guardian ad litem.

  • Public or mass media outreach

• Share only appropriate, respectful, and relevant media content
• Avoid publishing location-identifying information in public materials
• Ensure all shared content aligns with program values and community standards

I, {parentguardianName}, the parent or legal guardian of {studentName}, understand that both parent/guardian and student responsibilities must be upheld to ensure a safe, respectful, and enriching environment.

1. Communication
   I will stay informed through the bi-monthly newsletter and contact Project Learn staff with questions or concerns.
2. Drop-Off and Pick-Up
   - Drop-off no earlier than 9:45 AM
   - Pick-up no later than 3:15 PM
3. Food and Supplies
   I will send my child with lunch, snack, water bottle, and necessary supplies. I will communicate any relevant allergies or health concerns.
4. Dress Code
   I will ensure my child follows the program’s dress code.
5. Technology Usage
   I will support the program’s no personal device use policy.
6. Cancellations and Holidays
   I understand that cancellations may occur due to weather, emergencies, or holidays, and make-up days may not be available.
7. Tuition
   
   • Tuition is due by the 1st of each month
   • Tuition is non-refundable and not prorated
   • I am responsible if my charter school does not pay
8. Curriculum
   I am responsible for ensuring my child completes the required core academic curriculum outside of Project Learn.
9. Behaviour and Conduct
   I will support respectful behaviour. Repeated misconduct may result in dismissal.
10. Withdrawal
    If withdrawing from the program, I will notify the Site Director promptly.

11. Behaviour & Conduct
    Student attends prepared, with a respectful attitude, and follows staff instructions.
12. Technology Usage
    No personal devices (including mobile phones) during program hours. Misused devices may be confiscated.
13. Dress Code
    Student will wear attire suitable for learning and active outdoor participation.

Acknowledgment of Tuition Responsibility

I, {parentguardianName}, the parent or legal guardian of {studentName}, accept full financial responsibility for all tuition and fees associated with participation in Project Learn.

I understand and agree that:

• Tuition is due on the 1st of each month, regardless of attendance
• Tuition is non-refundable and not prorated for missed or unused days
• If my charter school fails to pay, I am fully responsible for tuition
• This agreement is effective for the full enrolment period
